Study On Risk Factors Affecting Recurrenceofpapillary Thyroid Carcinomas Of Tumor Size Less Than Or Equal 2cm

ObjectiveTo explore risk factors affecting recurrence of papillary thyroid carcinomas of tumor size less than or equal 2cm, tumor diameter 1-2cm and less than or equal 1 cmMethodsBetween Jan 2008 and Dec 2010 in PLA General Hospital,323 eligible patients received first surgery and diagnosed pathologically with papillary thyroid cancer?2cm were analyzed retrospectively. According to tumor size, patients were divided into PTMC and PTC of 1-2cm, which were investigated recurrence factors.ResultsFinally we indentified 320 PTC of tumor size less than or equal 2cm,218(68.1%) were PTMC and 102(31.9%) were PTC of 1-2cm. Median length of follow-up was 72.5(55-90) months. Of total patients,32 (10%) of experienced recurrence, including 22 cases(10%) in PTMC and 10 cases(9.8%) in PTC of 1-2cm. In the clinical characteristics analyses, larger PTCs (1.1-2.0cm) had more frequently high-risk age(less than 30 years and over 60 years), lymph node metastasis and TNM stage when compared with PTMCs. The univariate analysis suggested that lymph node metastasis, tumor foci, extrathyroidal extension and surgical method were associated with recurrence of papillary thyroid carcinomas of tumor size less than or equal 2cm. Lymph node metastasis influenced recurrence of PTMC and PTC of 1-2cm, while tumor foci and extrathyroidal extension were risk factors of recurrence in PTMC and surgical method only influenced recurrence of PTC of 1-2cm. According to Cox multivariate analysis, recurrence was independently associated with tumor foci and lymph node metastasisSurgical method and lymph node metastasis were respectively independent factors influencing the recurrence of papillary thyroid carcinomas of tumor size of 1-2cm and less than or equal 1 cmConclusionTumor foci and lymph node metastasis were independent factors which influenced the recurrence of papillary thyroid carcinomas of tumor size less than or equal 2cm. Risk factors affecting recurrence node metastasis and surgical method were respectively independently associated with recurrence.
