Objective: by comparing the changes in S100 B Protein and caspase-3 level in neonatal umbilical cord blood of neonates born to gestational diabetes mothers and normal full-term neonates,and the relationship with the neonatal behavior neurological assessment(NBNA) scores, explore the correlation and possible mechanism of S100 B and caspase-3 level and brain damage of neonates born to gestational diabetes mothers,for the early diagnosis of brain damage of neonates born to gestational diabetes mothers, providing basis for treatment and reducing the long-term sequelae.Methods: Choose86 neonates delivered in the First People's Hospital of Lianyungang between January 2015 and January 2016, gestational age37-42 weeks, singletons, prenatal without intrauterine distress, suffocation, intrauterine infection, congenital genetic metabolic disease, congenital heart disease, or anemia.All neonates are divided into two groups on the basis of motherhood with and without gestational diabetes: 1?Gestational diabetes group(experimental group):33 cases of motherhood in the diagnosis of gestational diabetes. 2?Normal pregnancy group(control group): 53 cases of full-term neonates in the same period with gestational age and motherhood without gestational diabetes. Collect 5 ml of umbilical vein blood sampling of all the neonates in 5 minutes after the birth, returned the serum after centrifugal processing in 30 minutes for examining S100 B and caspase-3 content. Monitor blood sugar of neonates of gestational diabetes group and normal control group after the neonatal birth for continuous three days, maintain blood sugar in normal range, and measure NBNA score on the third day. This experiment adopts double antibody sandwich enzyme-linked immunosorbent(ELISA) method to test S100 B protein and caspase-3 levels in neonatal umbilical cord blood of the two groups,measures NBNA score by professional child care physician on the third day, and the data results in average ± standard deviation(?? ±SD), according to statistical analysis by SPSS 18.0 software.Adopt repetitive measure to analyze variance, analyze correlation using linear correlation, and shows significant differences with P < 0.01Results: 1.Basic information of gestational diabetes group and control group neonates: Gestational diabetes groups: pregnant female age(30.03±3.80) years, gestational age(38.26±0.96) weeks, gender(18 male 15 female), birth weight(3779.09±640.34)g. Control group: pregnant female age(30.35±3.42) years, gestational age(39.62±1.02) weeks, gender(24male 29 female), birth weight(3447.74±352.68)g. The statistical test: There is no obvious difference between the two groups in pregnant female age, gestational age, sex, and birth weight(P > 0.05). 2.S100 B protein levels in neonatal umbilical cord blood of gestational diabetes group and control group: Two groups have significant difference(P<0.01): gestational diabetes group(38.80±9.28 pg/ml) and control group(12.3±2.56 pg/ml). 3. Caspase-3 level in neonatal umbilical cord blood of gestational diabetes group and control group: Two groups have significant difference(P<0.01): gestational diabetes group(5.32±1.67 ?g/ml) and the control group(2.83±0.55?g/ml). 4.NBNA comparison of gestational diabetes group and control group: Two groups have significant difference and statistical significance(P<0.01). Gestational diabetes group(36.27±1.23 scores), the control group(38.34±1.16 scores). 5.Correlation analysis of S100 B and caspase-3 of gestational diabetes group and control group and NBNA score: Gestational diabetes groups: S100 B was negatively correlated with NBNA, correlation coefficient 0.583,((P<0.01).Caspase-3 was negatively correlated with NBNA scores, correlation coefficient 0.814,(P<0.01). S100 B was positively related with caspase-3, correlation coefficient of 0.394,(P<0.05).Control group: S100 B, caspase-3 and NBNA score,There were no correlation between each other,(P>0.05).Conclusion: 1. S100 B and caspase 3 in neonatal umbilical cord blood of neonates born to gestational diabetes mother have higher protein expression, NBNA score measured on the third day reduced, which hint the gestational diabetes mother newborns possible have brain damage, associated with neurological dysfunction. 2.S100 B protein and caspase-3 levels of neonates born to gestational diabetes mothers were negatively correlated with neonatal NBNA score. S100 B protein and caspase-3 were positively correlated, suggesting S100 B protein and caspase-3 can be used as an important reference index predicting the degree of brain damage. 3.Motherhood gestational diabetes is probably one of the risk factors of neonatal brain damage. 4.Neonatal intrauterine of gestational diabetes mother causes chronic hypoxic brain damage.
