Objective:To investigate the expressions of Ki-67,Topo?a,Her-2 before and after the neoadjuvant chemotherapy in primary breast cancer,and to analyze the relationship between various molecular subtypes,the above indicators and the effect of neoadjuvant chemotherapy.Methods:We selected 150 patients with histological diagnosis of primary breast cancer in the second affiliated hosipital of dalian medical university between January 2014 to June 2015.All of the patients were confirmed by clinical imageology and needle biopsy as ??? clinical stage primary invasive breast carcinoma.The Ki-67,ER,Topo ? a,PR and Her-2 of breast cancer tissue were detected by immunohistochemistry technique.The patients received three or four cycles of neoadjuvant chemotherapy(TE regimen): docetaxel(75mg/m2)+ epirubicin(75mg/m2)with an interval 21 days.Then the patients underwent surgical treatment.The above indicators of resected breast carcinoma tissue were detected by immunohistochemistry technique again.Then we collected the statistical data and made it into corresponding forms.Because the data conformed to normal distribution,we used the chi-square test,at the same time,we analyzed the correlation between indications by Spearman test.Results:After three or four cycles of neoadjuvant chemotherapy,the number of patients with pathological complete response(pCR),complete response(CR),partial response(PR),stable disease(SD)and progressive disease(PD)were 4(2.67%),7(4.67%),107(71.33%),32(21.33%)and 0(0.00%).The total efficient(RR=pCR+CR+PR)was 78.67%.There were 39(26.00%)patients with Luminal A,68(45.33%)Luminal B,29(19.33)Her-2,14(9.34%)Triple negative breast cancer.The neoadjuvant chemotherapy efficiency of various molecular subtypes breast cancer were Luminal A(82.05%),Luminal B(83.82%),Her-2(68.97%),Triple negative breast cancer(64.29%).The expression percentage of Ki-67 and Topo ? a posttreatment were lower than prior-treatment(P<0.05);The effective rate with high expression for Ki-67 and Topo?a positive were higher than low expression for Ki-67 and Topo?a negative(P<0.05);The effective rate with reduced expression for Ki-67 was higher than no-reduced expression for Ki-67(P<0.05);The effective rate with reduced expression for Topo?a was lower than no-reduced expression for Topo ? a(P < 0.05);The positive rate of Her-2 expressions before and after the neoadjuvant chemotherapy was not statistically different(P>0.05);The effective rate with different expression of Her-2 and molecular subtypes breast cancer were not statistically different(P>0.05).There was a positive correlation between Topo?a and Ki-67(rs=0.426,P=0.000);There was no correlation between Topo?a and Her-2(P>0.05);There was a positive correlation between Ki-67 and Her-2(rs=0.372,P=0.000).Conclusions:Ki-67 and Topo?a can effectively evaluate the efficacy of neoadjuvant chemotherapy(TE regimens),they can be reliable bases for choosing the effective neoadjuvant chemotherapy regimens in treatment of breast cancer.
