Objective:To explore the level of uric acid and renal vascular lesions in IgA nephropathy,to analyse Relationship between uric acid and renal vascular lesions and provide a certain amount of clinical evidence to delay progression of IgA nephropathy.Methods: 60 patients with primary IgA nephropathy who diagnosised with clinical and renal biosy were enrolled in this study in our hospital between August 2014 and Februrary 2016.According to histological characteristics by light microscope,the patients were divided into two groups: renal vascular lesion 32 patients and no renal vascular lesion 28 patients.Collect all of the patients' gender,age,systolic blood pressure,diastolic blood pressure,blood glucose,total cholesterol,triglyceride,albumin,blood urea nitrogen,creatinine,estimated glomerular filtration rate(e GFR),urine protein and blood uric acid;to evaluate the pathology with The Oxford classificationof IgA nephropathy.(1)Compare the situation of baseline in renal vascular lesion group and no renal vascular lesion group;(2)Compare the situation of uric acid difference in renal vascular lesion group and no renal vascular lesion group;(3)Compare the occurrence rate of hyperuricemia in the two group;(4)Relationship between uric acid and renal vascular lesions;(5)Multiple variance regression analysis between uric acid and renal vascular lesions;(6)Compare the pathological score difference in the two group;(7)Relationship between pathological score and renal vascular lesions;(8)The ROC curve of uric acid in the diagnosis of renal vascular lesion.Results:(1)Compare the situation of baseline in renal vascular lesion group and no renal vascular lesion group.No significant differences between renal vascular lesion group and no renal vascular lesion group were found in gender,age,diastolic blood pressure,blood glucose,total cholesterol,triglyceride,albumin,blood urea nitrogen,creatinine(P>0.05);significant differences were found in systolic blood pressure,albumin,e GFR and urine protein(P<0.05).systolic blood pressure and urine protein is lower,albumin and e GFR decreased.(2)Compare the situation of uric acid difference in renal vascular lesion group and no renal vascular lesion group.Significant differences were found in uric acid,it is decreased in renal vascular lesion group(P<0.05).(3)Compare the occurrence rate of hyperuricemia in the two group.The occurrence rate of hyperuricemia is decreased in renal vascular lesion group(P<0.05).(4)Relationship between uric acid and renal vascular lesions.The relationship between uric acid and renal vascular lesions are positively correlated(P<0.05).(5)Multiple variance regression analysis between uric acid and renal vascular lesionsThe multiple variance regression analysis showed that uric acid and renal vascular lesions independent influencing factors of renal vascular lesion in the two group(OR=1.013,P<0.05).(6)Compare the pathological score in the two group.Significant differences were found in glomerular,global/segmental and Interstitial fibrosis in the two group(P<0.05).(7)Multiple variance regression analysis between pathological score and renal vascular lesions.The relationship between glomerular,global/segmental and Interstitial fibrosis are positively correlated(P<0.05).(8)The ROC curve of uric acid in the diagnosis of renal vascular lesion.The area of the ROC curve is 0.788,95%CI(0.662-0.894),(P=0.000),so uric acid in the diagnosis of renal vascular lesions has a certain reference value.With 342umol/L for diagnostic point,the sensitivity is 0.750,the specificity is 0.714;with 368umol/L for diagnostic point,the sensitivity is 0.625,the specificity is 0.821.Conclusions:(1)IgA nephropathy patients with renal vascular lesion of clinical and pathology are heavier.(2)The relationship between elevated uric acid and renal vascular lesions are positively correlated.(3)Uric acid in the prediction of renal vascular lesions has a certain reference value.
