Background:severe acute pancreatitis?severe acute pancreatitis,SAP?is one kind of abdominal critically ill clinical disorder,is caused by various causes,many factors involved in,with the complexity of the disease,rapid progression,more complications,poor prognosis and high mortality rates,has been receiving increasing attention in clinical medical professions.Currently,Alprostadil is still controversial in the treatment of SAP,and alprostadil on literature analysis and evaluation of clinical treatment SAP is still less.Therefore,this study will to investigate the clinical efficacy of alprostadil in the treatment of early SAP analysis.Objective:Observation of the safety and clinical efficacy of alporstadil in the treatment of severe acute pancreatitis,and to provide the basis for comprehensive treatment of early clinical SAP.Methods:The study selected 121 pantients with severe acute pancreatitis from2010.09-2014.09 in the second affiliated hospital of nanchang university.All the pantients were randomly divided into the treatment group?used alporstadil?58 cases and the untreatment group?unused alporstadil?63 cases.Patients were recorded of the time,age and gender on admission,After admission,with the development of the treatment procedures,we checked the patients within the 1d?7d?14d for ET-1 and PAF,APACHE-?score and CT severity index?CTSI?score,serum procalcitonin?PCT?,C-reactive protein?CRP?,Ca2+,alanine aminotransferase?ALT?,creatinine?Cr?level and other related indicators,collected and processed data to statistical analysis in order to evaluate the clinical efficacy of the use of the impact of alprostadil treatment of SAP.Results:?1?Two groups of patients on admission for age,gender were no significant difference?P>0.05?;two groups of patients admitted to hospital on the1 day ET-1 and PAF,CTSI score and APACHE-? score,serum PCT,CRP,Ca2+,ALT,Cr were no significant difference?P>0.05?;?2?Compared with the 1day,two groups of patients at the 7d and 14 d time points ET-1 and PAF levels,CTSI score andAPACHE-? score,serum PCT,CRP,Ca2+,ALT,Cr levels were significantly lower,with statistically significant?P<0.05?;?3?Compared with the control group,the treatment group at the 7d and 14 d time points ET-1 and PAF levels,CTSI score and APACHE-? score,serum PCT,CRP,Ca2+,ALT,Cr levels were lower than the control group,with statistical significance?P<0.05?;?4?there are 4 patients required surgical treatment in the treatment group and 6 patients required surgical treatment in the control group,the two groups showed no significant differences in surgery?P>0.05?;two groups each with three patients died and the mortality were no significantly difference?P>0.05?.Conclusion: 1?Alprostadil has assisted synergistic therapeutic effect in SAP in early treatment.2?alprostadil adjuvant treatment of SAP,can inhibit the release of ET-1 and PAF vasoactive substances,reduce the CTSI score and APACHE-II score,reduce the level of serum PCT and CRP,raising the concentrations of serum Ca2+,and play a protective effect on the liver and kidney function,but surgery and mortality was no significantly improvement.
