ObjectiveThis study through immunohistochemical method(SP method) to detection nasal polyps tissues and normal inferior turbinate mucosa of palate,lung,nasal epithelium clone(PLUNC),Toll-like receptor 2(TLR-2) and nuclear factor-Kappa B(NF-κB) expression, Analysis of correlation of their expression, exploring PLUNC, TLR2, NF-κB mechanism in nasal polyps, providing a new method for the treatment of nasal polyps. Materials and Methods46 cases of nasal endoscopic resection of nasal polyps specimens as the experimental group, 19 cases in the same period for nasal septum slants diorthosis in normal inferior turbinate mucosa as control group. HE dyed to determine the expression EOS and others inflammatory cells, PLUNC、TLR2 and NF-κB in the expression and distribution of the experimental proup and control group with immunohistochemistry. To compare the positive expression of PLUNC、TLR2 and NF-κB between polyps group and control group, and to analyze their relevance. The expermental data from all use SPSS21.0 software for processing. Results1. The experimental group after staining showed epithelia proliferation thickening, cilia disorder odging, edema, obviously stratified squamous epithelium to replace part of pseudostratified ciliated columnar epithelium, and obviously the epithelium and stroma in a large number of inflammatory cells infiltration, give priority to with EOS infiltration, including EOS, neutrophils, plasma cells, mononuclear and lymphocytes, and macrophages, etc. Control group mucosal surface with relatively flat pseudostratified ciliated columnar epithelium, lamina propria or interstitial for loose elastic fiber, connective tissue, a small amount of EOS and other inflammatory cells infiltration. Nasal polyps group compared with control way visible nasal polyps EOS infiltration was significantly higher than in normal inferior turbinate group(P<0.05).2. PLUNC in experimental group and control group are visible expression, positive expression parts throughtout mucosal epithelium and submucosal glands of cell membrane and cytoplasm, yellowish-brown, tan. The experimental group weakly positive expression in 11 cases(11/46), 5 cases are positive expression(5/46), the positive rate of 34.78%(16 cases/46); Control group is weakly positive in 2 cases(2/19), positive expression in 11 cases(11/19), strongly positive in 4 cases(4/19), positive rate of 89.74%(17 cases/19). Compared with control group and experimental group, the expression level of PLUNC in experimental group is significantly lower, there is a statistical significance(P < 0. 05).3. TLR2 are visible expression of different degrees in the experimental group and the control group, positive expression is mainly in nasal mucosa epithelium, the lower the gland in the cytoplasm and cell membrane, matrix layer and individual inflammatory cells in the lamina propria, its present tan or(and) brown. Weakly positive expression in the experimental group 8 cases(8/46), 13 cases of positive(13/46), strongly positive in 16 cases(16/46), positive rate is 80.43%(37 cases/46); Weakly positive in 3 cases in control group(3/19), the positive rate of 15.79%(3 cases/19).The expression of TLR2 in nasal polyps group is obviously higher than the control group, with statistical significance(P < 0.05).4. NF-κB in the experimental group mainly expressed on the glandular epithelial cells, endothelial cells, mast cells, monocytes and mucosa epithelial cells or(and) in the cytoplasm and nucleus, present tan or brown, the control group has also expressed. Experimental group weakly positive expression in 5 cases(5/46), positive expression in 11 cases(11/46), 20 cases of strong positive(20/46), the positive rate of 78.26%(36 cases/46); The control group, 4 cases are weakly positive(4/19), positive rate of 21.05%(4 cases/19). Positive rate between the two groups was statistically difference(P < 0.05).5. Spearman correlation analysis showed that PLUNC in nasal polyps group is negatively correlated with TLR2(r =-0.675, P < 0. 05), PLUNC is negatively correlated with NF-κB(r =-0.500, P < 0. 05), TLR2 is positively correlated with NF-κB(r = 0.540, P < 0. 05). EOS infiltration degree positive correlation with TLR2 and NF-κB exist,(r=0.417, 0. 470, P < 0.05),degree negative correlation with PLUNC exist(r=-0.859,P<0.05). Conclusions1. PLUNC, TLR2 and NF-κBp65 visiable expression in normal inferior turbinate tissues, suggesting that nasal sinus mucosa exist innate immune defense system, the three may be involved.2. PLUNC expressed in nasal polyps tissues is lower than the normal inferior turbinate tissues, suggesting that the formation of nasal nolyps may be associated with lower natural immunity, increase the expression of PLUNC may’s new strategy for development intervention in nasal polyps.3. Nasal polyps tissues TLR2 and NF-κBp65 positive expressed is higher than normal inferior turbinate tissues, prompting infectious factors may be an important factor in development and incidence of nasal polyp, both high expression may promote the occurrence of nasal polyps.4. PLUNC in nasal polyps tissues is negatively correlated with the expression of TLR2 and NF-κBp65,suggesting that there may exist in PLUNC-TLR2-NF-κB reaction shaft in nasal polyps, the reaction shaft may play an important role in the development of the occurrence of nasal polyps, the drugs that inhibit the reaction shaft, which may bring new ideas for the treatment of nasal polyps.
