The Clicinal Effect Of Different Surgical Treatment Of Laryng Otracheal Stenosis

Objective By collecting the clinical data of surgical treatment of laryngotracheal stenosis patients in recent years, to understand the formation of laryngotracheal stenosis causes and clinical diagnosis,to summarize the clinical effect and related factors of different surgical treatment of laryngotracheal stenosis, and provide reference for clinical treatment and prevention of laryngotracheal stenosis.Methods The clinical data of 59 patients from July 2006 to July 2015 were analyzed. Of all 59 cases,including 7 enfants and 52 adults, 10 cases underwent CO2 laser resection,5 cases underwent temperature controlled radiofrequency ablation and 5 cases received laryngeal mold under self-retaining laryngoscope,6 cases received Keel placement after laryngofissure,25 cases received T-shape tube implantation after laryngoplasty or tracheoplasty and 8 cases underwent end to end tracheal anastomosis after cervical tracheotomy. All the patients had been followed up for 6 months to 9 years postoperatively.Results40 of 59 were cured after single-operation,12 of 59 were cuerd after Repeated operations,no one suffered from laryngotracheal restenosis again.5 cases failed and were surviving with tracheal tube. 2 cases lost to follow-up.No people die or have complications.Among all the cases,5 cases with laryngeal mold and 8cases with tracheal anastomosis were cured after single-operation.Among 10 cases laser scar removl surgery,6 cases were cured after single-operation,3cases were cured after the second operation,1 case of laryngeal cancer recur;Among 5 cases with temperature controlled radiofrequency ablation,3 cases were cured after an operation,1 case were cured after two operations,1 had no effect;Among 6 cases with Keel placement,4 cases were cured after an operation,1 case were cured after two operations,1 had no effect; Among 25 cases with T-shape tube implantation,14 cases were cured after an operation,7 cases were cured after two and more operations,2 cases had no effect and 2 cases were lost.Conclusion In order to improve the therapeutic effect of laryngotracheal stenosis,the treatment strategy should be personalized according to the degree of stenosis,the position,the impairment of laryngotracheal framework and so on.
