Background and objectiveBreast cancer is one of the most common malignant tumors in women, is aslo evident heterogeneity of tumorsin the malignant tumor, At the same time its morbidity and mortality among the women tumor is the first, about 120-150 thousands of New breast cancer cases worldwide each year, about 50-57 thousands ofcases of deaths, serious threat to women's physical and mental health.Neoadjuv antchemotherapy is milestone event for breast cancer Comprehensive therapy, it is the role of the standard treatment of locally advanced breast cancer has been accepted by the scholars both abroad and at home, has been widely used in clinical treatment. Neoadjuvant chemotherapy,Also known as preoperative chemotherapy,refers to distant metastasis of malignantthe tumor,first give cytotoxic drugs for the treatment of systemic systembefore localized treatment(radiation therapy or surgery).The study found that the neaoadjuvant chemotherapy can reduce the size of primary tumor and reduce the clinical stage, increases the opportunity of breast-conserving surgery, reduce the spread of the tiny metastatic lesions and individual good predicting the sensitivity and effectiveness of chemotherapy regimens.AlsoThe disease-free survival(DFS) and overall survival(OS) of patients who can get pathologic complete response(p CR)by neaoadjuvant chemotherapy can be improved obviously. But many clinical studies have found that not all breast cancer patients could benefit from the neoadjuvant chemotherapy, some even bring the toxic effects of chemotherapy, and other local treatment of delay or cause local progress.So Predicting the curative effect of neoadjuvant chemotherapy effective factor research has been a hotspot in current researches.Snail and Ecadherin express in neoadjuvant chemotherapy curative effect the predictive value of this research is less.This topic will befurther study about it, by collecting 169 cases of in patients undergoing neoadjuvant chemotherapy, use hollow needle puncture beforeneo adjuvant chemotherapy of breast cancer tissue, by immunohistochemical SP method to detect the expression of Snail and Ecadher, discusses the joint test to study the curative effect of neoadjuvant chemotherapy and prognosis, which have a positive impact of this disease. MethodCollect 169 cases of speciments of stage ?~ ? breast cancer tissue after core needle puncture operation from the patients who were comfirmed in the pathology department of the first affiliated hospital of Zhengzhou university from January,2008 to june 2012. Immunohistochemical assay was used to detect the snail and E-cadherin expression level of 169 breast cancer tissues obtained by core needle biopsy before neoadjuvant chemotherapy.The relationship of snail and E-cadherin expression with clinicopathologicalcharacteristics and efficacy with5 years survival situation as well as prognosis of neoadjuvant chemotherapy was statistically analyzed. Results1. In 169 cases of breast cancer tissues, there are 6 cases of tissue samples in no expression,the positive expression rate is 96.4%, among which 99 were highly expressed, 70 cases of lower expression. there are 10 cases of tissue samples in the expression of no E-cadherin, positive expression rate is 94.08%;2. The expression of Snail was related with the lymph node metastases, clinical stage,tumor size(p<0.05), The expression of E-cadherin was related with the clinical stage,histological, lymph node metastases,(p<0.05);there is no significant relevant between the expression of Snail and E-cadherin and age,the menstrual status,and the expression of ER?PR.3. In the 169 cases of patients,there are 51 cases achieved Snail low expression group, 45 cases achieved the CR and PR in high expression, the clinical total effective rate of low expression group(CR and PR) are significantly higher than high expression group(72.86% VS 45.45%, P = 0.000). in the E cadherin low expression group of 169 patients, 68 cases of patients reached the CR and PR, 45 cases achieved the CR and PR in high expression group, the clinical total effective rate of E-cadherin low expression group(CR and PR) are significantly higher than E-cadherin high expression group(78.16% VS 54.88%, P = 0.001).4. Snail and E-canherin expression in breast cancer organizations exist negative correlation(r =-0.241, P = 0.002).5. Kaplan-meier Survival curves show thatthe five survival rates between the Snail and E-cadherin low expression group and high expression of comparative differences are statistically significant(P < 0.05).And Snail lower expression group is higher than than the high expression of group of the survival rate.E-cadherin high express is higher than the lower expression of group of the survival rate. Conclusions1. There were higher expression of Snail in breast cancer, there were low expression of E-cadherin in breast cancer;2. The expression of Snail was related with the lymph node metastases, clinical TNM stage, ER and PR expression(p<0.05), The expression of E-cadherin was related with the clinical TNM stage,histological, lymph node metastases, ER and PR expression(p<0.05); there is no significant relevant between the expression of Snail and E-cadherin and age,the menstrual status,and the expression of ER?PR.3. The expression of Snail and E-cadherincan be as an effective predictors of neoadjuvant chemotherapy of breast cancer.
