The Clinical Research On The Traditional Chinese Medical Treatment Of Premature Thelarche Girls

ObjectiveTo summarize professor Wang Shaojie's clinical treating experience of premature thelarche,that on the base of improving diet and lifestyle,combined with the characteristics of northern coastal areas patients,apply nourishing Yin and removing fire,soothing liver and strengthening spleen and reducing phlegm and resolving masses by different syndrome to treat this disease.Observe and evaluate the clinical effect of the method and explore its mechanism to provide an effective treatment for improving patients' clinical symptoms and reducing central precocious puberty conversion.Method90 cases were from the Dalian Children's Hospital during March 2014 to March 2015,aged 3-7 years old.They were randomly divided into observation group and control group of 45 cases each.There is no statistically significant difference(P > 0.05)among two groups of children with age,height,weight,breast development stage ect,so they are comparable.Control group: improve their diet and lifestyle.Observation group: on the base of improving diet and lifestyle,apply zhibai dihuang wan adding or subtracting.Medicine: anemarrhena,phellodendron,radix rehmanniae,yam,poria,cortex moutan,concha arcae,bupleurum,radix paeoniae alba,radix pseudostellariae and licorice,3-6g each.One dose daily,two times per day.One course of the treatment is 3 months,generally 1 to 2 courses,no other drugs during treatment.The two groups were followed up for 6-12 months.During the treatment,Breast development stage and traditional Chinese medicine symptoms and signs recorded every 2 weeks;height,weight,breast ultrasound recorded every month;uterine and ovarian ultrasound recorded every 3 months.After the treatment,breast development stage,traditional Chinese medicine symptoms and signs,height and weight recorded every 3 months;the changes of ultrasound imaging(breast,uterus and ovary)recorded every 6 months.Bone age recorded every 6 months.Hormone levels and gonadotropin releasing hormone stimulation test before the treatment and rechecked 3-6 months when necessary.At the same time,monitor the clinical adverse reaction of the observation group.Use IBM SPSS Statistics 19.0 to analyse the data.Result1.In 90 cases,yin deficiency and fire hyperactivity type is most common,41.1%(37 cases);The types of pathogenic fire derived from stagnation of liver-QI and phlegm dampness are 32.2% and 26.7%.2.The total effective rate of the observation group is 88.9%(25 cured,15 cases improved),and the control group is 68.9%(10 cured,21 cases improved),the observation group surpasses the control group,and the difference is statistically significant(P < 0.05).3.traditional Chinese syndrome measurement value comparison: before treatment,two groups had no statistically significant difference(P>0.05).After treatment,both groups decrease,the differences are statistically significant(P<0.01);compared between groups,the observation group decreases more significantly than the control group,the difference is statistically significant(P<0.01).4.Central precocious conversion rate: the observation group is 2.2%(1 case),and the control group is 13.3%(6 cases).5.After treatment,the observation group showed no obvious abnormalities in blood and urine routine,stool routine,electrocardiogram,liver and renal function.During follow-up,there were no obvious adverse reactions in two groups.Conclusions1.The main type of the premature thelarche girls is yin deficiency and fire hyperactivity type.2.Summarize professor Wang Shaojie's clinical treating experience of premature thelarche,and suggest on the base of improving diet and lifestyle,combined with the characteristics of northern coastal areas patients,applying nourishing Yin and removing fire,soothing liver and strengthening spleen and reducing phlegm and resolving masses by different syndrome to treat this disease.3.The dialectical,comprehensive treatment,combined with prevention,can promote the extinction of breast mass,effectively improve the clinical symptoms and signs and reduce the central precocious puberty conversion rate.It is safe and effective,worthy of promotion.4.For the long-term effect of this therapy in premature thelarche girls need further study.
