Objective:To provide the basis for the clinical diagnosis and treatment of the disease,this research discuss the clinical characteristics of diabetes with lung abscess.Though this way,it is hoped that this disease is well to known,appropriate treatment is chosen,the rate of missed diagnosis and misdiagnosis is reduced and the treatment efficiency is improved.Methods:Using retrospective analysis,this research collect and sort the clinical data of 172 patients with lung abscess who came to the First Affiliated Hospital of Da Lian Medical University from January 2008 to November 2015.Patients were divided into two groups:diabetes mellitus group and non—diabetes mellitus group.The two groups were analyzed constituent ratio ?the sex distribution and average age.Diabetic patients with lung abscess were analyzed distribution characteristics of different ages.According to whether suffering from diabetes,the patients who have complete data were divided observation group and control group.The observation group and the control group were compared from general materials,basic diseases,clinical symptoms,laboratory examination,etiology examination and imaging findings.At the same time,therapy and treatment effect in diabetic patients with lung abscess was summarized.Results:Among 172 patients with lung abscess,diabetic patients was present in 30%(52/172).In non—diabetes mellitus group,the percentage of women is only 13.3%,while in diabetes mellitus group it is 26.9%.It suggests that the prevalence of female in diabetes mellitus group was significantly higher than that in non—diabetes mellitus group.There were significant differences between Observation group and control group in basic diseases,including hypertension(32% VS15.6 % P = 0.018).The main clinical symptoms of two groups were fever,cough,yellow sputum or white.There were no statistic differences between the observation group and the control group in laboratory examination,including white blood cell count? percentage of neutrophils?anemia and serum level of albumin.The positive rate of Klebsiella pneumonia in observation group are higher(23.8% %VS7.5% p=0.008).Abscess is often appearanced in the right lung and alone.But abscess in observation group are more likely to be multiple(26% VS10.1 % p = 0.009).The image findings also show that abscess in observation group are more likely to occur in the bilateral lung(22% VS7.3 % p = 0.008),while in the control group are more likely to occur in the lower right lobe(27.5% VS12 % p = 0.030).In diabetes mellitus group,the main treatment are positive control of blood glucose,timely control of infection?postural drainage?actively treating basic diseases and control of other complications,etc.The total effective rate in this group is 88.5%.Conclusion: 1.The clinical presentation of lung abscess in diabetic patients were atypical,which can lead to missed diagnosis and misdiagnosis.2.Compared with non-diabetic patients with lung abscess,the diabetic patients with lung abscess were prone to have hypertension disease and klebsiella pneumoniae infection.For the diabetic patients with lung abscess,abscess are more likely to be multiple and occur in the bilateral lung.
