Objective:The purpose of this paper is to analyze the dynamics of renal function,explore the correlation between the changes in renal function and the early prognosis of hypoxic ischemic encephalopathy(HIE)with varying levels of severity and provide experimental basis for HIE clinical treatment.Methods:The method adopted is prospective case-control study.The subjects selected are the 60 sick children admitted into neonatal intensive care units(NICU)at the Military General Hospital of Beijing and diagnosed with HIE between January,2014 and January,2016.The control group was comprised of 20 healthy neonates born during the same period at the same hospital.Those newborns included 51 boys and 29 girls,with gestational age ranging from 37 weeks to 42 weeks and birth weight between 2500 g and 4700 g.The subjects were divided into three groups according to HIE severity: mild HIE group,moderate HIE group and severe HIE group,with each comprised of 20 subjects.The control group which had 20 healthy neonates matched the subjects according to gender,gestational age,birth weight and delivery modes.The subjects' renal function were examined on Day one and Three after hospitalization,includingserum creatinine,serum urea nitrogen and blood uric acid tests,as well as observing a EEG.The magnetic resonance imaging(MRI)examination and neonatal behavioral neurological assessment(NBNA)were undertaken on Day 14,using observing table to track clinical data.The statistical software package SPSS19.0 was employed for data analysis,with ±S indicating measurement data,using one-way analysis of variance and independent-sample T test to analyze.The enumeration data were verified by ?2,test.The normal distribution and homoscedasticity were verified by F test.If p<0.05,the difference was deemed as significant.Results:1.There was significant difference between the control group and the mild HIE group in creatinine values(Day One and Day Three).It was the same case with among different HIE groups.The general trend was the severe HIE group>the moderate group>the mild group>the control group.When comparing the creatinine values(Day One)of any two groups,there was significant difference between the control group and the mild group,the control group and the moderate group,the control group and the severe group,the mild group and the severe group(p<0.05).2.The creatinine values(Day One)of sick children with different prognoses showed significant difference(p<0.05).3.There was significant difference among the control group,the moderate HIE group and the mild HIE group in urea nitrogen values(Day One and Day Three).It was the same case with among different HIE groups.The general trend was the severe HIE group>the moderate group>the mild group>the control group.When comparing the urea nitrogen values(Day One)of any two groups,there was significant difference between the control group and the mild group,the control group and the moderate group,the control group and the severe group,the mild group and the severe group(p<0.05).4.The urea nitrogen values(Day One)of sick children with different prognoses showed significant difference(p<0.05).5.There was significant difference among the control group and the moderate HIE group in uric acid values(Day One and Day Three).It was the same case with among different HIE groups.The general trend was the severe HIE group>the moderate group>the mild group>the control group.When comparing the uric acid values(Day One)of any two groups,there was significant difference between the control group and the mild group,the control group and the moderate group,the control group and the severe group,the mild group and the severe group(p<0.05).6.The uric acid values(Day One)of sick children with different prognoses showed significant difference(p<0.05).7.There was significant difference in the severity of renal injury among patients with varying levels of HIE severity(p<0.05).8.There was significant difference in the severity of renal injury among sick children with different prognoses(p<0.05).Conclusion: 1.The severity of renal injury has good relevance with HIE severity and early prognosis.2.The severity of renal injury had relevance with HIE severity.In the severe HIE group,the more severe the renal injury is,the more likely the acute renal failure is to happen.3.The renal function indicators such as serum creatinine,blood urea nitrogen and blood uric acid vary according to HIE severity.4.Serum creatinine,blood urea nitrogen and blood uric acid can be used as indicators of early treatment and prognosis of mild and severe HIE.
