A Comparative Study On Laparoscopic Asssited Distal Gastrectomy For Gastric Cancer Between Billroth-? And Billroth?+Braun

Objective:To explore the clinical efficacy of the Billroth ? combined Braun and conventional Billroth? assited by laparoscopic in the treatment of distal gatric cancer.Method: Choose in our hospital in March 2014-February 2016 treated 80 cases of patients with gastric cancer,according to the digital random table method,80 patients were divided into observation group and control group(40 cases in each group);Among them,the observation group,the male 18,female 22,the average age(69.4±9.8),height(167.75±7.5)cm and weight(67.40±5.5)kg,TNM staging cases: ? period in 16 cases,? period in 16 cases,8 ? period;16 control group 24 of the men and women,average age(71.2±8.8),height(168.25±8.5)cm and weight(71.97±5.9)kg,staging gastric cancer cases: ? period 18 cases,? period 14 cases,8 cases of ? period.Observation group of patients with surgical ways for laparoscopic assisted distal gastric cancer radical Billroth ? + Braun anastomosis,control group patients with surgical ways for laparoscopic assisted distal gastric cancer radical Billroth-? anastomosis,all patients were accepted D2 lymph node clearing operation.Compare two groups of operation time,input loops in patients with syndrome,bile reflux gastritis,duodenal stump fistula,incidence of postoperative complications such as gastric paralysis syndrome,the incidence of anastomotic fistula,intraoperative blood loss,recovery time of gastrointestinal function,length of hospital stay.Results: Laparoscopic assisted distal gastric cancer radical Billroth ? + Braun anastomosis group input loop syndrome incidence,bile reflux gastritis,duodenal stump fistula incidence in obvious lower than conventional Billroth laparoscopic-? anastomosis group,the difference had statistical significance(P < 0.05);Billroth ? + Braun and anastomosis of Billroth-? anastomosis of the incidence of anastomotic fistula patients,gastric paralysis syndrome incidence,intraoperative blood loss,postoperative ventilation time and hospital stay compared differences had no statistical significance(P > 0.05),but Billroth ? + Braun anastomosis groups of operation time than conventional Billroth-? leader,with statistical differences.Conclusion: Laparoscopic downward Billroth ? + Braun anastomosis gastric cancer treatment,safe and feasible.Can effectively reduce the input loop syndrome,bile reflux gastritis,duodenal stump fistula in incidence,although surgery operating time is relatively long,but can improve the quality of life of patients with postoperative,has the high promotion value in clinic.
