Objective: To investigate the clinical & epidemiological features of femoral intertrochanteric fracture in elderly patients referred to the eight hospitals in district of Beijing,Tianjin,Tangshan,and provide evidence for treating and preventing this fracture.Methods: Eight hospitals were included in the current study,of them 6 hospitals(Tianjin Hospital;Peking university third hospital;Beijing Chaoyang hospital;Beijing Shijitan hospital;Wangjing hospital of CACMS;The central hospital of China Aerospace Corporation)were classified as level ? and the other 2 ones(Luhe hospital of Tongzou Beijing;The second hospital of Tangshan)were level ?.The clinical DRs data and basic informations of patients over 60 years with femoral intertrochanteric fractures admitted to the 8 hospitals of Beijing,Tianjin,and Tangshan from January 2010 to December 2011 were reviewed from the picture archiving and communication systems(PACS)or the taken films and analyzed retrospectively.All the data including patients' registered number,X-ray photo ID,date examination,gender,age,combined fracture,AO and Evans types were abstracted and analyzed to investigate the epidemiologic characteristics of intertrochanteric fractures occurring during this time in the districts.Exclusion criteria: old fractures,rib fractures,reexamination and fractures of skull et al.Relevant reviewers were trained to classify the fractures,including AO and Evans classification.If any disagreement was encountered,a discussion was made until a consent was reached.Quality control was needed during the investigation,training the staff,stressing the signicance and importance of the survey,requiring seriously and carefully,being proficient the elements and precautions of the survey forms.Two professional doctors regularly checked the quality of these work.When the misjudgment rate for the fracture diagnosis was more than 1% or the fracture classification error rate was more than 3%,the staff had to do the work again of the same part.Results: A total of 1583 adult intertrochanteric fracture patients were included,Beijing 491 cases,Tianjin 293 cases,799 cases of Tangshan,643(40.6%)males,940(59.4%)female(M:F=1:1.46).Fractures involving left side were slightly more than the right(825VS758).The fracture was most common in 71~80 years old(45.5%),the mean age was(77.6±7.6)years.31-A2(61.1%)was the most popular fracture type in AO classification system.Evans?was more common in this group(27.4%),followed by II,III,IV,V(26.3%,22.1%,12.7%,11.5%).Conclusion: Intertrochanteric fracture was most common in females and in age of 71~80 years which accounted for 51.9% of all elderly hip fractures and 73.4% of all intertrochanteric fractures in adults in three cities.31-A2 and Evans?was the dominant fractures type for AO and Evans classification systems,respectively.
