X-ray And CT In The Diagnosis Of Femoral Intertrochanteric Lateral Wall Fracture Classification Evaluation Study

Objective:Femoral intertrochanteric fractures were classified to AO classification,Evans-Jensen classification and Gotfried classification by X-ray and CT.The authenticity and reliability are analyzed,To analyze the advantages and disadvantages of two kinds of imaging methods in the diagnosis and classification of femoral intertrochanteric fractures,and to guide the correct surgical treatment.Methods:In this study we collected 66 cases of intertrochanteric fractures,in time for the November 2011 to June 2017,their preoperative X-ray and CT data integrity.Under the leadership of a senior physician,By using two kinds of imaging data,Each case was classified to AO classification,Evans-Jensen classification and Gotfried classification.The results were compared with the clinical results,and the reliability and validity of the results were statistically analyzed.Results:AO typing of A2.1,A2.2,A2.3 and A3.1,Evans-Jensen typing of II?III?IV?V,Gotfried typing of I?II?III.The sensitivity of CT diagnosis is higher than or equal to X-ray diagnosis,The rate of missed diagnosis is lower than or equal to X ray diagnosis,the specificity is higher than or equal to X ray diagnosis,the misdiagnosis rate is lower than or equal to X ray diagnosis,the coincidence rate is higher than that of X-ray diagnosis.The results showed that the ability of CT to correctly detect cases and exclude non cases was better than that of X-ray.AOA2.1 type X-ray kappa value =0.57,the moderate trusted.CT kappa value =1,which belongs to the fully trusted.A2.2 X-ray kappa =0.32,belonging to mild to moderate trusted.CT kappa value =0.92,which belongs to the fully trusted.A2.3 X-ray kappa=0.86.CT kappa,belonging to the fully trusted value =0.86,belongs to the fully trusted.A3.1 the kappa value of X-ray =0.37,belonging to mild to moderate trusted.CT kappa value =0.92,is completely credible.Evans-Jensen type II X-ray kappa value =0.51,the moderate trusted.CT kappa value =1,which belongs to the fully trusted.III X-ray kappa =0.27,belonging to mild to moderate trusted.CT kappa value =0.94,which belongs to the fully trusted.IV X-ray kappa =0.62,belonging to the basic trusted.CT kappa = 0.96,entirely trusted.V X-ray kappa value=0.51,the moderate trusted.CT kappa value =0.93,is completely credible.Gotfried type I type X-ray kappa =0.69,belonging to the basic trusted.CT kappa value=0.97,which belongs to the fully trusted.II X-ray kappa =0.77,belonging to the basic trusted.CT kappa value =0.85,which belongs to the fully trusted.III X-ray kappa =0.61,belonging to the basic trusted.CT kappa value =0.84,is completely credible.
