| Objective: Compare the clinical repair effect on ankle joint deltoid ligament injury by treatment or not in order to provide reference for the selection of clinical treatment methods.Methods: A retrospectively analysis was introduced in 53 patients with ankle fracture of the third department of the first affiliated hospital of Dalian Medical University.34 male and 19 female,the youngest is 19 and the oldest is 60,33 on average.53 cases with ankle fracture were divided into two groups,28 cases were treated with the repair of the deltoid ligament(group A),and 25 cases were treated with deltoid ligament not be repaired(group B).Patients were all followed up for 12 months to 15 months after operation.Operation time,intra-operative bleeding,length of stay,healing time,the clearance of medial malleolus and talus inclining angle in preoperative and postoperative were analyzed and compared retrospectively.The complications include ankle instability,traumatic arthritis and so on.The ankle joint function evaluation for two groups by AOFAS ankle scoring system.Data collection and significant analysis of all follow-up and review were carried by SPSS19.0 software.Results: No wound infection,blood vessel and nerve injury occurred during operation.Of 28 cases of the repair of the deltoid ligament(group A),the average duration of operations is 65.O±10.2min,the average intra-operative bleeding is 80±22.4ml,Of 25 cases of deltoid ligament not be repaired(group B),the average duration of operations is 44.0±13.4min,the average intra-operative bleeding is 50±20.6ml.The mentioned difference have no statistical sense(P>0.05).The type of fracture,length of stay and healing time have no statistical sense(P>0.05).The A group does not appear any complications,B group has ankle instability in 3 cases,1 cases of traumatic arthritis.There were significant differences in the incidence of postoperative complications between group A and B.The data of MCS of the two groups by within-group comparison:The differences of MCS between preoperative and postoperative(in stress free position)were statistically significant in both groups(P < 0.05).The differences of MCS between preoperative and postoperative(in stress position)were statistically significant in both groups(P < 0.05).The differences of MCS between stress free position and stress position in postoperative were statistically significant only in B group(P < 0.05).The differences of MCS between one year after operation and postoperative(in stress position)were statistically significant only in B group(P <0.05).The results by between-group comparison:it has statistical significance only in the differences of MCS in postoperative(in stress position)and differences of MCS in one year after operation between the two groups(P < 0.05).The data of TT of the two groups by within-group comparison:The differences of TT between preoperative and postoperative were statistically significant in both groups(P < 0.05).The differences of TT between postoperative and one year after operation were statistically significant only in B group(P < 0.05).The results by between-group comparison:the differences of TT in postoperative and differences of TT in one year after operation has statistical significance(P < 0.05).12 months after operation,the ankle joint function score A group average 94.66+2.26,B group average 83.67+3.42,AOFAS ankle score by statistical analysis showed that there is a difference(P < 0.05)between the two groups.Conclusions: There were no significant differences in length of stay and blood loss between the two groups of A and B in groups.Ankle joint fracture with deltoid ligament injury,fracture reduction and internal fixation with repairing the triangle ligament in the same time can reduce the complications after ankle surgery.The repair of the deltoid ligament is good for talus to maintain the anatomical location,and the strength of the repaired ligament is higher,which can provide better stability of ankle joint.The function of ankle joint in group A was better than that of group B in 12 months after operation.It is necessary to treat the patients in a timely manner when we deal with ankle joint fracture with deltoid ligament injury. |
