Analysis On The Case Report Of Removable Partial Denture Using Long Occlusal Rest

Objective Through the improvement of dentition defect in patients with prosthetic design,make the patient resume occlusal function,decreasing abutment lateral force,protect the periodontal ligament,delay absorption of alveolar bone,improve the service life of the prosthesis.Methods A total of seven patients(male 3,female 4,age from 55 to 67,average age61)with defect dentition are obtained from Dalian Stomatological Hospital affiliated to Dalian Medical University,form July 2014 to December 2015.Among them,two cases of patients with Kennedy Class II dentition defect,four cases of dentition defect of Kennedy Class III patients(two case of Kennedy III class the first sub-category),one case of mandibular Kennedy I class with Kennedy Class III maxillary dentition defect patients.Although the 7 cases of patients with defect position is not the same,but will residual elongation of tooth,unopposed teeth or movement to the gap lateral tilt phenomenon.Therefore,conventional retention design not only can not improve these problems,which will further aggravate the abutment torsional stress.So,in this article,the long occlusal rest is placed in the absence of side gap of the abutment teeth,because the denture occlusal force is through the occlusal rest to the abutment of the transitivity,therefore,by extending rest length improve abutment stress direction and the basis teeth can along the long axis of the tooth side to force,to protect abutment.Meanwhile,in selecting the appropriate retainers on the basis of the denture but also to ensure balance and stability.According dentition defect cases were 7 patients choice planar or vertical line supports two kinds of ways to make a good prosthesis retention and stability,restore the function of dentition defects.Repair facial morphology observed before whether patients symmetry,occlusion,temporomandibular joints and whether the opening of normal,asked whether the systemic disease,whether worn removable partial dentures,the patient understand the remaining teeth oral health,alveolar bone is formed,there is no significant bone spurs and ridges,redness,swelling and mucous membranes inside the mouth hygiene.Then inform patients restorative treatment programs,as well as the repair process and repair problems that may arise will be described later,the last patient informed consent.Performed before surgery in case of patient consent,intraoperative and postoperative photographs.In the collected cases,the design of the denture is the retention,balance and stability of the denture.Retainer number not less than two and not more than four,supporting the selection of long occlusal rest,snap clasp using a circular clasp.The good root canal filling abutment selection using magnetic attachment,choice vitallium or cobalt chromium alloy stent,extension of the flange moderate fully,artificial tooth selection type plastic tooth anatomy.Results Treatment cycle ended a week 2 cases after follow-up adjustment the occlusal,and requires patients respectively in repair after 3 month,6 months and one year after return visit to observe the repair effect,which 1 case because of physical reasons,can not be timely referral,and the other 6 cases without occlusion discomfort,temporomandibular joint and so on.The patient is easy to wear,no mucosal tenderness,swelling and other problems,such as tooth movement,chewing function and facial features have improved,the restoration effect is more satisfactory.Conclusion Removable partial denture is one of the commonly used methods of repairing dentition defect,for the poor condition of abutment,lack of tooth number,free deletion cannot be grown repair and reluctance to wear in addition to dental patients ismore suitable method.And application of long occlusal rest not only increase prosthesis retention force can improve the abutment stress situation,while healthy root reserves in favor of the presence of periodontal ligament proprioception,in conjunction with magnetic attachment,so reducing side abutment to stress,thereby reducing the periodontal ligament damage and prolong clinical abutment there is a time,and thus delay the absorption of alveolar bone.
