The Clinical Experience Of The Casting Occlusal Surface Removable Partial Denture For Dentition Defect With Inadequate Bite-gum Space

Objective: By observing clinical effect of the casting occlusal surface removable partial denture for the different types of dentition defect with inadequate bite-gum space,to discussing the restorative methods of dentition defect for the inadequate bite-gum space,and to improving the success rate of restoration.Methods:28 cases were selected from Jan.2014 to Oct.2015 who with dentition defect and bite-gum space was less than 6mm.There were 17 males and 11 females among these patients;the age was from 45 to 80;average age was 60 years old.4 cases were light and moderate mix,9 cases were light and serious mix,moderate and serious mix were 2 cases,13 cases were serious.The patients whowith light inadequate bite-gum space used plastic base and resin teeth removable partial denture.The moderate dentition defect used plastic base and metal occlusal surface removable partial denture.The serious dentition defect used whole metal casting removable partial denture.According to the different situation with the corresponding method of treatment.Cases criteria:? Dentition defect with inadequate bite-gum space,no obvious decrease of vertical distance and inconformity fixed denture indications or patients rejecting high cost of orthodontic implant.? Abutment no loosening or loosening degree less than I.?No disease of the jaw,mucous membrane and TMJ.(4)No patients with limb disorders and who cannot manage life.Observing and evaluating the clinical effect of restoration after 1 to 21 months.Evaluation criterion: good: patients withoutsubjective discomfort.The denture is solid,stable,with high chewing efficiency,abutment teeth without pain.Base with abutment teeth and mucous membrane have no obvious gap.Mucous membrane have no inflammation and ulcer.For the patients who decrease of vertical distance,improving surface type,no TMJ discomfort.Poor: masticatory efficiency reducing ?loosing abutment?dentures retention is poor?abutment and clasp are broken.There is a significant gap with base and mucous membrane caused by remaining food,TMJ discomfort.There is no satisfactory results after adjustment.Results:By the clinical follow-up observingof 28 cases which used casting occlusal surface removable partial denture in 1 to 21 months,26 patients without subjective discomfort,the denture is solid?stable?easy to wear,and good masticatory efficiency,abutment teeth without pain or loose.Mucous membrane have no inflammation and ulcer.Denture base plate is closely attached with abutment teeth and mucous membrane have no obvious gap.No TMJ discomfort.Effective rate is 92.9%.Two cases are poor,non efficiency rate is 7.1%.One case of cobalt chromium alloy stent whose inadequate space is severe mixed light,it's clasp was broken.The other one with more seriously inadequate space whose abutment teeth with pain and loose.Mucous membrane occurs ulcer.After adjusting and buffering,the effect is not obvious.Conclusion:The restoration method of casting occlusal surface removable partial denture can improve the chewing efficiency effectively and prolong the service life of the denture for the dentition defect with inadequate bite-gum space.
