Urokinase Micropump Combined Shenxiong Glucose Injection Treating Arteriosclerosis Obliterans

Lower extremity arteriosclerosis occlusive disease of is a common disease,frequently occurred in surgical department.The disease is part of systemic atherosclerosis.It is caused by angiostegnosis and vascular occlusion,which is related to atherosclerosis.The disease is the main reason of lower extremity ischemia,also,important risk factor of disabilities,or even death.The lesions mainly involve the femoral,popliteal artery,followed by the tibia,the peroneal artery,and patients with diabetes,with extremities artery firstly and mainly suffered.In recent years,due to the influence of people's bad habits,change in diet,life pressures and population aging and other factors,the incidence rate showed a rising trend,and it's a serious threat to people's health.With the continuous development of medical equipment and technology,treatment of arteriosclerosis obliterans has been rapidly developed,but at home and abroad,there is still no effective cure to this disease.This study attempts to look for a more effective treatment method to control arteriosclerosis obliterans from the integrative medicine perspective.Objective: to investigate the clinical value of treating lower extremity arteriosclerosis occlusive disease of blood stasis syndrome patients with the urokinase micro pump and Shenxiong glucose injection.Methods: The study choosed 60 patients,who has a clear diagnosis of arteriosclerosis obliterans of blood stasis syndrome,and who had been hospitalized in the Integrative ward of the Dalian Medical University Affiliated Second Hospital from March 2014-October 2015.They were randomly divided into two groups,30 in each group.The control group were given 0.9 Nacl 50 Ml + Injection Urokinase 500,000 u,1 times/day,10mL/h with the micro pump;while the treatment group were given Shenxiong glucose solution 200 ml added,patients with diabetes mellitus,with insulin 2u,1 time/day intravenous infusion added.The treatment time was 14 days.Corresponding symptomatic treatment were given to patients who with diabetes mellitus,coronary heart disease,high blood pressure.If the limb was accompanied by ulcers,for patients whose ulcer area is small,topical ointment was given;for patients whose ulcer area is large,or accompanied by local or systemic infection,symptomatic treatment of corresponding antibiotics intravenous drip was given.For handling of wound,erode debridement was taken as principle.When obvious boundaries appeared between necrotic tissue and healthy tissue,we completely clear away the necrotic tissue.After necrotic tissue was thoroughly removed,closed negative pressure drainage and?or?dot skin grafting was taken if patient's condition is appropriate to promote wound healing.We compare the data of the pain,numbness,cold sensation,intermittent claudication?individual normal speed?,ulcer healing?area?,etc,before and after treatment between the control group and the treatment group.The we comparatively analyzed the transcutaneous oxygen pressure?TcPo₂?and ankle / brachial pressure index?ABI?of the limb before and after treatment.Results:1.All the patients' blood indicators,blood coagulation indicators,urine,stool routine,liver function,renal function were not found abnormally changed before and after treatment.2.There is no statistical significance between the control group and the treatment group in age,sex,clinical course after statistical treatment?P>0.05?.3.Before treatment,there is no statistical significance between the control group and the treatment group in foot and leg transcutaneous oxygen pressure?TcPo₂?,ankle brachial index?ABI?,and symptom score after statistical treatment?P>0.05?.4.After treatment,foot and leg transcutaneous oxygen pressure?TcPo₂?,ankle brachial index?ABI?,and symptom score after statistical treatment of the treatment group are all statistical significanlly better than the control group?P<0.05?.In the treatment group,clinical cure 10 cases,markedly effective 14 cases,effective 5 cases,ineffective 1 case,totally effective 29 cases?96.67%?.In the control group,clinical cure 3 cases,markedly effective 10 cases,effective 9 cases,ineffective 8 case,totally effective 22 cases?73.33%?.The total curative effect of the treatment group was significantly better than that of the control group?P<0.05?.Conclusion: For arteriosclerosis obliterans of blood stasis syndrome patients,the use of micro-pump combined with urokinase Shenxiong glucose injection therapy achieved exact effect,and is worthy of clinical application and generalization.
