Clinical Study On The Expression Of Leptin And Adiponectin In The Treatment Of Non-alcoholic Steatohepatitis With Hepatitis Treatment Capsule No.3(HTC3)

Objective: To observe the expression of leptin and adiponectin in the treatment of non-alcoholic Steatohepatitis with HTC3(syndrome of stagnant dampness-heat),and to explore and confirm the therapeutic effect and mechanism of this drug on non-alcoholic Steatohepatitis.Methods: Based on the principle of random,single blind and positive parallel control,60 cases of patients with non-alcoholic fatty hepatitis(damp heat stasis syndrome)were randomly divided into two groups,i.e.treatment group and control group,30 cases in each group,the treatment group take the HTC3,the control group was treated with Tiopronin tablets.The treatment lasted for12 weeks.Liver and spleen CT value ratio as the criterion for the liver fat degree measurement for the patients with NASH,have correlation analysis of the measured leptin,adiponectin,body mass index(BMI),TBI,alt,AST,GGT and TG,TC,HDL-C,LDL-C and other experimental data,get related factors of the liver and fat degree.By observing the CT value ratio of liver and spleen,changes of TCM syndrome score,liver function recovery rate,TC,TG,BMI before and after treatment and the expression of leptinand adiponectin during the treatment process,to further clarify the therapeutic effect of HTC3 to NASH,clarify molecular pathological mechanism the treatment of NASH and to explore theoretical basis for more in-depth research to NASH treatment.Results: the severity of fatty liver is obviously negatively correlated with the GGT(r =-0.567,P < 0.001),TG(r =-0.322,P = 0.012),TC(r=-0.322,P=0.012),Leptin(r=-0.276,P=0.033),BMI(r=-0.977,P < 0.001),positively correlated with HDL(r=-0.658 P < 0.001),adiponectin(r=0.978,P < 0.001);serum adiponectin levels are obviously negatively correlated with GGT(r=-0.567,P < 0.001),TG(r =-0.459,P<0.001),BMI(r=--0.471,P< 0.001): positively correlated with HDL(r=0.467,P < 0.001),CT and ratio(r=0.623,P<0.001);serum leptin levels are obviously negatively related with BMI(r=-0.275,P=0.047),and CT ratio(r=-0.276,P = 0.033);positively correlated with ALT(r = 0.298,P =0.021).For CT ratio of liver and spleen of the two groups of patients,the treatment group is better than the control group(P=0.04<0.05);For TCM syndrome classification quantitative integral,the treatment group is better than the control group(P=0.036<0.05);As for the level of adiponectin,in the treatment group(P<0.01)and control group(P<0.01),the level of adiponectin is comparatively improved.Compare the two groups,the group in which the patients taking HTC3 is better than the group taking tiopronin(P = 0.018<0.05);For leptin,HTC3 group(P=0.018 < 0.05)can improve the level of serum leptin,while no significant improvement of serum leptin level to tiopronin group(P=0.147>0.05);For the recovery rate of liver function,HTC3 isequivalent to tiopronin(P=0.432 > 0.05);For the success rate of TG,HTC3 group is better than tiopronin group(P=0.035< 0.05);For BMI,the level of quantitative comparison,HTC3 is also better than sulfur Pronin(P = 0.037 < 0.05).Conclusion:Severity of fatty liver is negatively correlated with GGT,TG,HDL,and BMI,is positively associated with LDL and adiponectin;serum adiponectin levels are negatively correlated with GGT,TG,and BMI,is positively associated with HDL and CT ratio of liver and spleen;Serum leptin levels is negatively correlated with BMI and positively correlated with ALT,liver and spleen CT ratio.Liver Treatment Capsule 3 can improve the adiponectin,leptin,CT ratio of liver and spleen,TCM syndrome integral,body mass index(BMI),recovery rate of liver function and TG rate of NASH patients,showing that HTC3 has therapeutic effect on NASH,and such therapeutic effect is likely to be achieved through regulation of serum leptin and adiponectin level.
