| Objective:After acute myocardial infarction?AMI?occurred,hypoxia and ischemia led to myocardial cells necrosis,the left ventricular remodeling?LVR?occured,if without prevention and intervention and allowed to continued,it would develop into heart failure,and seriously affect the patients' quality and safety of life,finally.Some studies showed that the concentration of visceral adipose tissue-derived serine protease inhibitor?vaspin?was correlated with coronary artery disease,and it was negatively correlated with the severity of coronary artery stenosis.The severity of coronary artery disease in patients was positively correlated with the left ventricular remodeling.The aim of this study was to investigate the correlation between serum vaspin level and LVR after AMI occurred.Methods:1 My study group included 77 adults from the Second Hospital of HeBei Medical University from January 2015 to January 2016,52 adults were diagnosed with AMI,25 adults were diagnosed with stable angina pectoris?SAP?.We refined the general situations of all the patients who were enrolled,including sex,age,height,weight,past medical history,family medical history,ect.At the same time,we excluded other organic heart diseases,blood system disease,respiratory system disease,endocrine system disease,malignant tumors,severe liver and kidney dysfunction,past heart failure,servere infection,pregnancy,lactationwomen,ect.Refining blood routine,urine routine,stool test,blood biochemistry analysis,myocardial enzyme,electrocardiogram for all the patients who were admitted by hospital.AMI group patients were examined by real time three dimensional echocardiography?RT-3DE?and serum vaspin at 1 week,1 month and 3 months,to obtaine the related indicators of left ventricular remodeling.During the follow-up period,all the AMI patients acceped oral drugs therapy,drugs including aspirin,clopidogrel,angiotension converting enzyme inhibitor?ACEI?/angiotensin receptor blocker?ARB?,?-blocker,ect.And then,serum vaspin concentration was measured by enzyme linked immunosorbent assay?ELISA?.2 Examination of real time three-dimensional echocardiography?RT-3DE?:The left lateral position was taken by patients.Measuring the left ventricular end systolic volume?LVESV?,left ventricular end diastolic volume?LVEDV?,left ventricular end diastolic epicardial volume?LVEDVepi?of subjects by the same doctor.According to the basis formula:LVRIproportion =(LVRI 1week or 1 month-LVRI x month)/ LVRI 1week or 1 month,calculating LVRIproportion from 1 week to 1 month,1 month to 3 months and 1week to 3 months.All data used statistical analysis system SPSS 22.0,P<0.05 have statistical difference,P<0.01 have significant statistical difference.Results:1 AMI group compared with the SAP group,the concentration of vaspin of AMI was lower than the SAP group.2 The serum vaspin concentrations were?0.86±0.15,0.14±0.99,0.19±1.19?ug/L at 1 week,1 month and 3 months.1 week lower than 1 month?P<0.05?,1 month lower than 3 months?P< 0.05?,1 week lower than 3 months?P < 0.05?.3 The LVRI?1 week1 month?proportion,LVRI?1month3 months?proportion,LVRI?1 week3 months?proportion of AMI group were?0.10±0.02,0.11±0.02,0.20±0.03?.LVRI?1 week1 month?proportion and LVRI?1 month3 months?proportion had no significant difference?P > 0.05?,LVRI?1 week1 month?proportion was lower than LVRI?1 week3 months?,LVRI?1 month1 month?proportion was lower than LVRI?1 week3 months?.4 The correlation analysis showed that the serum vaspin concentration was negatively correlated with LVRIproportion,linear regression analysis showed that the concentration of vaspin and LVRIproportion were independently correlative.Conclusions:1 The concentration of serum vaspin in AMI group was lower than SAP group.2 After AMI occurred,the concentration of serum vaspin increased in a certain time range.3 After AMI occurred,the concertration of vaspin and LVRIproportion were negatively correlated,and according to the concentration of serum vaspin to predict LVRIproportion and the severity of LVR,providing important information for the prediction and intervention of LVR. |
