Study On Early TCM Syndrome In Patients After Gastric Cancer Surgery

1 BackgroundGastric cancer is one of the most common malignant tumor in our country, in traditional Chinese medicine can be called stomachache, nausea, dysphagia,accumulation and so on.Gastric cancer always appear in the age of 50 years of age or older,the incidence ratio between male and female is about 2:1.stomach cancer is a serious threat to the people's health as it's mortality rate is very high. At present the most effective method treatment of gastric cancer is use surgical treatment, but because of the normal gastrointestinal movement and digest function changed for the digestive tract reconstruction after stomach surgery, and the surgical trauma make the body's immune function fall more poor. That can lead the residual tumor more easily growth and metastasis, and produce a variety of complications, which affects the recent recovery and long-term survival rate. Traditional Chinese medicine have effects of improve the symptoms of postoperative gastric cancer, reduce a variety of complications, promote the body function recovery, control and eliminate cancer, maintain and restore the function of the body, which can improve the quality of life after surgery and prolong the survival time. This study aims to explore the early TCM syndrome in patients after gastric cancer surgery and guide clinical treatment with those results.2 ObjectiveThis research was to use the method of clinical epidemiology to statistics the early TCM syndromes in patients after gastric cancer surgery, summarizes the main syndrome and minor syndrome in early postoperative gastric cancer, analysis the distribution regulation of TCM syndromes. Research the relationship between the TCM syndrome and gender, age, tumor pathologic type, tumor pathologic installment, surgical methods.Enrich the content of the early TCM syndrome differentiation in patients after gastric cancer surgery, and to establish a scientific system of early TCM syndrome differentiation in patients after gastric cancer. To provide theoretical basis for medication and further research in early period after gastric cancer surgery.3 MethodsWe collect the 182 postoperative gastric cancer patients that accord with Diagnosis from Department of general surgery, the First Affiliated Hospital of Anhui traditional Chinese Medicine University during January 2013 to December 2015.Typing patients in different TCM syndrome,collect patients' gender, age, tumor pathologic type, tumor pathologic installment, operation methods,at the same time; Analyze and compare the relationship between the TCM syndrome and patients' gender, age, tumor pathologic type, tumor pathologic installment and operation methods in early period after gastric cancer surgery4 ResultsThrough study 182 cases of early postoperative patients with gastric cancer, we can draw a conclusion that the patients of gastric cancer after surgery always with multisyndrome, which on the basis of a main syndrome with on one or more secondary syndrome.4.1 The patient own two TCM syndromes at the same time after gastric cancer surgeryThrough research the 182 cases of gastric cancer patients' syndrome of TCM, the patient own two TCM syndromes at the same time have 92 cases, 50.55 percent of the research objects; spleen deficiency syndrome + fu viscera excess syndrome have30 cases, 16.48 percent of the research objects; The patients of spleen deficiency syndrome + blood deficiency syndrome have 9 cases, 4.95 percent of the research objects; The patients of fu viscera excess syndrome + temper deficiency syndrome have32 cases, 17.58 percent of the research objects; The patients of fu viscera excess syndrome + damp-heat syndrome have 6 cases, 3.29 percent of the research objects; The patients of blood deficiency syndrome + temper deficiency syndrome have 5cases, 2.75 percent of the research objects; The patients of Yin deficiency syndrome + temper deficiency syndrome, have 4 cases, 2.20 percent of the research objects; The patients of damp-heat syndrome + fu viscera excess syndrome have 3 cases,1.65 percent of the research objects; The patients of blood stasis syndrome + fu viscera excess syndrome have 3 cases, 1.65 percent of the research objects.4.2 The patient own three or more TCM syndromes at the same time after gastric cancer surgeryThrough research the 182 cases of gastric cancer patients' syndrome of TCM, the patient own three or more TCM syndromes at the same time have 90 cases, 49.45 percent of the research objects; spleen deficiency syndrome + fu viscera excess syndrome + blood deficiency syndrome have 25 cases, 13.74 percent of the research objects; The patients of spleen deficiency syndrome + fu viscera excess syndrome +blood deficiency syndrome + damp-heat syndrome have 20 cases,10.99 percent of the research objects; The patients of fu viscera excess syndrome + temper deficiency syndrome + damp-heat syndrome have 29 cases, 15.93 percent of the research objects;The patients of fu viscera excess syndrome + temper deficiency syndrome+ damp-heat syndrome + blood stasis syndrome have 10 cases, 5.49 percent of the research objects;The patients of fu viscera excess syndrome + damp-heat syndrome + Yin deficiency syndrome have 4 cases, 2.20 percent of the research objects; The patients of blood stasis syndrome + temper deficiency syndrome + blood stasis syndrome, have 2 cases,1.10 percent of the research objects;4.3 The early main syndrome in patients after gastric cancer surgery,There have six main syndrome in patients after gastric cancer surgery, including spleen deficiency syndrome, fu viscera excess syndrome,blood deficiency syndrome,Yin deficiency syndrome, damp-heat syndrome, blood stasis syndrome?The patients of spleen deficiency syndrome have 84 cases, 46.15 percent of the research objects. The patients of fu viscera excess syndrome have 81 cases, 44.50 percent of the research objects. The patients of blood deficiency syndrome have 7 cases, 3.85 percent of the research objects. The patients of Yin deficiency syndrome have 4 cases, 2.20 percent of the research objects. The patients of damp-heat syndrome have 3 cases, 1.65 percent of the research objects. The patients of blood stasis syndrome have 3 cases, 1.65 percent of the research objects. In which,the main syndrome is spleen deficiency syndrome and fu viscera excess, in total of 165 cases, 90.65 percent of the research objects. We can find that the main early syndrome is spleen deficiency syndrome and fu viscera excess in patients after gastric cancer surgery, other syndromes are relatively few.4.4 The appear frequency of the early main TCM syndrome in patients after gastric cancer surgeryThrough analyzing, the patients' main syndrome or minor syndrome belong to spleen deficiency syndrome have 166 cases, 91.21 percent of the total study patients;The patients' main syndrome or minor syndrome belong to fu viscera excess syndrome,have 161 cases, 88.46 percent of the total study patients. The patients' main syndrome or minor syndrome belong to spleen deficiency syndrome and fu viscera excess syndrome have 146 cases 80.22 percent of the total study patients.Through analyze of the above data, we can find that the appear frequency of spleen deficiency syndrome and fu viscera excess syndrome is very high, and they always appear together.4.5 The main factors of distribution of the main TCM syndrome in patients after gastric cancer surgeryThere on correlation between the TCM syndrome and gender, tumor pathologic type, tumor pathologic installment, surgical methods. There was no statistically significant difference between them(P>0.05). There have correlation between the TCM syndrome and age, the difference was statistically significant(P <0.05).5 ConclusionThe people over the age of 50 were more frequently occurring Gastric cancer, and the male was more frequently than female. Pathological types are mainly adenocarcinoma, pathological installment mainly focused on ?, ?, suggesting that most patients is too late to accept treatment,they always belong to middle-late gastric cancer patients. The results show that there have six main syndrome in patients after gastric cancer surgery, including spleen deficiency syndrome, fu viscera excess syndrome,blood deficiency syndrome, Yin deficiency syndrome, damp-heat syndrome,blood stasis syndrome. The main early syndrome is spleen deficiency syndrome and fu viscera excess in patients after gastric cancer surgery, other syndromes are relatively few.The appearance of TCM syndrome is relation to the age of patients. There always several syndromes appear together, in which spleen deficiency syndrome and fu viscera excess syndrome were most. This conform that the early TCM syndrome in patients after gastric cancer surgery is most deficient and excessive syndromes. deficiency is spleen deficiency syndrome, excess is fu viscera excess syndrome.
