Clinical Significance In Lung Cancer Diagnosis Of Combined Detection Of Serum TK1,CEA,NSE,CYFRA21-1 And CA125

Objective:In clinical,serum tumor markers has been widely used in auxiliary diagnosis for lung cancer,because there was non tumor marker which has both sensitivity and specificity for lung cancer.The single detection of tumor marker existed a certain degree of limitation,so the combined detection of tumor mark become the research hotspot in recent years.The aim of this paper was to investigate the single or combined level of serum TK1,CEA,NSE,CYFRA21-1 and CA125 in patients with lung cancer.We analysed the clinical significance for early diagnosis of lung cancer.Methods:Blood was took from 130 patients with lung cancer group,122 control group(including 41 benign and 81 Normal physical examination)from the Puren Hospital Affiliated to Wuhan University of Science and Technology between May.2014 and Nov.2015,which meet the inclusion and exclusion criteria.Immunoblotting enhanced chemiluminescence was used to detect serum TK1,and CEA,NSE,CYFRA21-1 and CA125 was measured by enzyme-linked immunosorbent assay(ELISA method).To compare various types of tumor markers in lung cancer test group and two control groups,and explore the relationship between various types of gastric cancer patients with serum tumor markers with sex,age,tumor stage,pathological type and lymph node metastasis.ROC curves were made with various types of tumor markers by SPSS17.0statistical software,and compared the area under the ROC curve,then determine the optimal cutoff value.By comparing the area under individual ROC curves and the sensitivity and specificity,which of tumor markers of all kinds of test group and the control group and joint detection(Combined detection contain TK1),to explore the clinical significance of combined detection of serum TK1,CEA,,NSE,CYFRA21-1 and CA125 in lung cancer.Results:These five tumor markers were significantly higher in gastric cancer test group than the benign control group and the healthy control group,and differences were statistically significant(P<0.05),the difference between benign group with the control group was not statistically significant(P>0.05);These five tumor markers were higher in stage ?patients than stage?,?,besides the TK1 was higher in stage ? patients than stage?,and the difference was statistically significant(P<0.05);These five tumor markers were higher in lymph node metastasis than those without metastasis,and the difference was statistically significant(P<0.05);The CEA,NSE,CA125 have auxiliary diagnostic significance for the pathological type,NSE concentrations in small cell lung cancer patients were significantly higher than those in non-small cell lung cancer,and CEA,CA125 concentrations in non-small cell lung cancer were higher than small cell lung cancer,the difference was statistically significant(P<0.05);The largest substance under ROC curve among single tumor marker was 0.897 in TK1,the sensitivity was 84.5%,and specificity was 72.9%;The highest sensitivity of two joint,which was up to 86%,was TK1+CA125 and TK1+CEA,and specificity was 78.2% and 80.8%,respectively;The highest sensitivity of three joint,which was up to 88.7%,was CEA+NSE+TK1,and specificity was 85.7%;The highest sensitivity of four joint,which was up to 93.8%,was CEA+CYFRA21-1+CA125+ TK1,and specificity was 92.4%;Five combined sensitivity of 96.7%,specificity was 87.4%,and AUC was 0.980;To identify the gastric benign and malignant lesions by the ROC curve,then determine the optimal cutoff value of TK1,CEA,,NSE,CYFRA21-1 and CA125,which were namely: 3.54ng/ml,10.89ng/ml,2.64ng/ml,20.46U/ml,1.42pmol/ml.Conclusion:TK1 is a good cancer marker for early diagnosis of lung cancer,but has no auxiliary diagnostic significance for the pathological type of lung cancer.The tumor markers CEA,NSE,CYFRA21-1,CA125 have a certain clinical value in the early diagnosis of lung cancer,and CEA,NSE,CA125 has auxiliary diagnosis for the pathological type of lung cancer.The sensitive of combined detection of tumor markers was significantly higher than the individual detections,however,when the index number is too much,the specificity will have a certain degree of decline,so,it is crucial to choose the appropriate combination of indicators.
