Objective:To explore the clinical diagnostic value of signal transducer and activator of transcription 3(STAT3) as a potential tumor marker(TM) by detecting the expression of STAT3 in blood of non-small cell lung cancer(NSCLC) patients.To provide theoretical basis for the application of STAT3 in NSCLC by analyzing the diagnostic value of combined detection with other related TMs.Methods:â‘ Real time PCR was applied to detect the expression of peripheral blood STAT3 mRNA in 10 NSCLC patients,10 benign pulmonary disease patients and 10 normal people. The ROC curve was used to analyze the clinical diagnostic value of STAT3 mRNA in NSCLC.â‘¡ELISA was applied to detect the expression of serum STAT3 protein,carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125) and cytokeratin 19 fragements(CYFRA21-1) in 40 NSCLC patients,40 benign pulmonary disease patients and 40 normal people. Analyzed the relationship between the expression level of serum STAT3 protein and the clinicopathologic characteristics of NSCLC.The ROC curve was used to analyze the diagnostic value of STAT3 protein and the accuracy of combined detection with other related TMs in NSCLC.â‘¢Applied one-way ANOVA to compare the difference of expression among groups.Applied the Z test to compare the difference of diagnostic value of detection targets. Results:â‘ STAT3 mRNA and STAT3 protein were expressed in all the groups,and the expression levels of STAT3 mRNA and STAT3 protein in lung cancer group were significantly higher than that of the benign pulmonary disease group and normal control group(P<0.01),the difference of the expression level of the benign pulmonary disease group and normal control group was not significant(P>0.05). The difference of the expression level of STAT3 protein was not significant in different histopathology type of tumor(P=0.574),lymph node metastasis(P=0.065) in NSCLC.However, the difference of expression level of STAT3 protein was significant in different degree of tumor differentiation(P<0.01), the level of poor differentiation was significantly higher than that of moderate differentiation and high differentiation(P<0.05),and the level of moderate differentiation and high differentiation had no significant difference(P>0.05).â‘¡The area under ROC curve(Az) of STAT3 mRNA and STAT3 protein were 0.870,0.860 respectively,the Az between two groups had no significant difference (P>0.05).â‘¢The Az of CEA,CA125,CYFRA21-1 were 0.78,0.788,0.780 respectively. The Az of STAT3 protein had no significant difference in contrast to other TMs(P>0.05).â‘£The Az of STAT3+CEA+CA125+CYFRA21-1 was 0.930, the Az of CEA+CA125+CYFRA21-1 was 0.841, the Az of four joint detection was significantly larger than that of three joint detection (P<0.05).Conclusions:â‘ STAT3 is expressed in the blood of NSCLC patients,benign pulmonary disease patients and normal people, moreover, STAT3 is overexpressed in NSCLC. The expression of STAT3 does not correlate with clinicopathological characteristics of NSCLC patients such as histopathology type of tumor, the state of lymph node metastasis However,the expression level of STAT3 correlates with the degree of tumor differentiation.â‘¡The diagnostic value of STAT3 and CEA,CA125,CYFRA21-1 is equivalent respectively and STAT3 may be become a new TM of NSCLC.â‘¢The diagnostic accuracy of STAT3+CEA+CA125+CYFRA21-1 in NSCLC is significantly higher than that of CEA+CA125+CYFRA21-1,the new joint detection method has higher diagnostic value in the early clinical diagnosis of NSCLC.
