Aim: Advanced researches showed that vitamin D combining the vitamin D receptor mediate anti-tumoural effects.Our experiments wanted to explore whether VDR mRNA involved in tumorigensis by comparing of VDR mRNA level in colorectal adenoma,colorectal adenocarcinoma and healthy volunteers;to check if vitamin D deficiency was a risk for colorectal adenoma and colorectal adenocarcinoma through comparing vitamin D level in plasma;to analyse the relationship between cilinical information and VDR mRNA and vitamin D.Method: We adopted case control study.Experimental sample came from human colorectal adenoma,colorectal adenocarcinoma and healthy volunteers.We employed real-time RT-PCR to examine the expression of VDR mRNA in tissue and PBMC and quantified results by using the 2^-??Ct or ?Ct method.We determined plasma vitamin D concentration by ELISA.Then,we combind clinical information and pathological files with VDR mRNA level and plasma vitamin D level to analysis their relationship.Result: 1.VDR mRNA expression was lower in colorectal adnocarcinoma than adjacent mucosa(P<0.01),VDR mRNA level was higher in healthy volunteers' colorectum than colorectal adenocarcinoma tissue(P<0.01)and colorectal adenoma tissue(P<0.01).There was no statistical difference between colorectal adenoma and colorectal adenocarcinoma(P>0.05);VDR mRNA level were not related to the degree of neoplasia,pathological pattern of adenoma,size et al.in adenoma;VDR mRNA level were not related to size,differentiated degree,PTNM,clinical stages et al.in adenocarcinoma.2.There was no statistical defference among VDR mRNA in PBMC which came from healthy volunteers,colorectal adenoma and colorectal adenocarcinoma(P=0.7).3.Vitamin D concerntration was much lower in patients with colorectal adenoma(P<0.01)and colorectal adenocarcinoma(P<0.01)than in healthy volunteers.low plasma vitamin D were significantly more likely to be a risk factor of colorectal adenoma(odds ratio(OR)4.875,95% confidence interval(CI): 2.144,11.084)and of colorectal adenocarcinoma(odds ratio(OR)2.975,95% confidence interval(CI): 1.364,6.271).4.We also found plasam vitamin D might be negative related to PBMC VDR mRNA level in healthy volunteers,but not in colorectal adenoma and colorectal adenocarcinoma.Conclusion: Low plasma vitamin D level might be the risk factor for colorectal adenoma and colorectal adenocarcioma.The reduction of VDR mRNA in adenoma and adenocarcinoma might be related to accurrence and development of colorectal adenoma and colorectal adenocarcinoma.But the rduction of VDR mRNA in adenoma and adenocarcinoma might be not related to adenoama's size,degeree of neoplasia,pathological type,nor related to adenocarcinoma's size,differentiated degree,PTNM,clinical stages.VDR mRNA in PBMC might have nothing to do with colorectal adenoma and adenocarcinoma.There was a negative correlation between plasama vitamin D concentration and PBMC VDR mRNA level in helthy volunteers,however,the correlation was absent in colorectal adenoma and colorectal adenocarcinoma.Above all,we show that low plasma vitamin D and decreased VDR mRNA in tissue were both related to colorectal adenoma and colorectal adenocarcinoma.Our study might provide a new clue for researching pathogenic mechanism of colorectal adenama and colorectal andadenocarcinoma.
