Urodynamic Study After Cervical Cancer Treated With Never-sparing Radical Hysterectomy

Objective:The patients of radical hysterectomy in preoperative and one month after surgery were performed for urodynamic examination.By comparing the effects of traditional radical hysterectomy and pelvic autonomic nerve sparing radical hysterectomy on bladder function to explore the advantages of nerve sparing radical hysterectomy.Methods: During the period from 2013 June to 2014 December in the gynaecology department of affiliated hospital of Wangnan Medical College,there were 31 cases of cervical cancer staged Ib1-IIA2 who underwent radical hysterectomy.They were performed randomized control study,including 16 cases of NSRH and 15 cases of RH.The patients of two groups had preoperative and postoperative urodynamic examination.This study has compared with the obtained data parameters for the analysis of statistically significance.Results: After one month of operations,all of the patients of two groups had underwent urodynamic studies.There were significant statistical differences in residual urine volume and maximum flow rate in the free flow rate measurement phase, P values respectively for 0.000 and 0.021.In the intravesical stage, the urgent urination sense, and first urination sense, and bladder maximum volume were increased in both groups, but two groups of NSRH and RH compared had statistics differences, with P values respectively for 0.019,0.032,0.041; when compared with two groups for bladder compliance, P value was 0.031.In the voiding phase, the maximum bladder pressure, maximum flow rate and maximum detrusor muscle pressure reduced in both groups, there were significant differences between the two groups, P values respectively for 0.013,0.010,0.014;meanwhile,in the group of RH,the increase of abdominal pressure was obviously, p<0.001.Conclusion: By comparing NSRH group and RH group shows that NSRH surgery can significantly reduce postoperative urinary retention, intermittent urination,lower urinary tract dysfunction and other symptoms of abdominal pressure urination.The recently results of urodynamic studies for patients of the two groups demonstrate that the injury of NSRH surgery to the lower urinary tract is lighter than RH surgery,which can protect the function of bladder sensation and detrusor muscle.
