Project1and2: The relationship between blood pressure multi-parameters andtarget organs damage in the elderly male with hypertensionObjective: To explore the target organs damage(heart?kidney?vessel) and anti-dipperhypertension in elderly male patients, accumulate the evidence of correlation betweentarget organ damage and rhythm of ambulatory blood pressure; To research the organdamage and risk factors in anti-dipper hypertension patients with diabetes, then try toprovide the clinical basis.Methods:650patients admitted to Chinese PLA General Hospital from2009to2012,were divided into369non-dipper hypertensive cases and281anti-dipper hypertensivecases.281anti-dipper hypertensive cases were divided into131diabetes mellitus casesand150no diabetes mellitus cases. Clinical data have been collected.Physical-biochemical indexes were measured.Ambulatory blood pressure monitoringand Heart and vessel ultrasonographys were performed.Results: Part1:The relationship between blood pressure multi-parameters andtarget organs damage in the elderly male with anti-hypertension1.98.3%of elderly male hypertensive patients had the change of blood pressure rhythm.55.8%of patients were no-dipper hypertension and42.5%of patients were anti-dipperhypertension.2.Compared with non-dipper hypertensive group, the age of anti-dipper hypertensivegroup was older; the TG,IVS,LVPW,LVEDD were higher in anti-dipper hypertensivegroup while hemoglobin?serum albumin?T3?LVEF was lower, and the incidence ofcoronary heart disease and?cerebral ischemia and carotid artery atherosclerosis was significantly higher.24hSBP, nSBP, nDBP were higher while dSBP, dDBP were lower.(all P<0.05).3.When adjusting for relative factors, the logistic regression analysis showed that age?morning DBP?CKD?LVMI?Morning-evening Difference were the risk factors ofanti-dipper hypertension,and whereas Pre-awaking Surge was the protective factor.(allP<0.05).4.When adjusting for relative factors, the logistic regression analysis showed that age?sUA?anti-dipper hypertension were the risk factors of CKD, and whereas HDL-C wasthe protective factor.(all P<0.05).5.When adjusting for relative factors, the logistic regression analysis showed thatmorning SBP?TC?LDL-C were the risk factors of Diabetes, and whereas HDL-C wasthe protective factor.(all P<0.05).6.When adjusting for relative factors, the logistic regression analysis showed thatmorning SBP?sCr?sUA?anti-dipper hypertension?Morning-evening Difference werethe risk factors of CAS.(all P<0.05).7.When adjusting for relative factors, the logistic regression analysis showed that age?anti-dipper hypertension?LDL-C were the risk factors of LVMI, and whereas HDL-Cwas the protective factor. The multiple linear regression analysis showed that LVMI waspositively correlated with NT-proBNP?age?CAD?TC and anti-dipper hypertension.(allP<0.05).Part2: The relationship between blood pressure multi-parameters and targetorgans damage in the elderly male with anti-hypertension and diabetes mellitus1.Compared with anti-dipper hypertensive group, the morning SBP, dSDP,TG,TSH,RAD were higher in anti-dipper hypertensive and diabetes mellitus groupwhile hemoglobin was lower, and the incidence of hyperurieemia(HUA) and lowerextremity artery atherosclerosis was significantly higher(all P<0.05).2.When adjusting for relative factors, the logistic regression analysis showed that age?morningSBP?TC?LDL-C were the risk factors of anti-dipper hypertensive and diabetesmellitus group, and whereas Rising Surge was the protective factor.(all P<0.05).3.When adjusting for relative factors,the logistic regression analysis showed thatNT-proBNP was the risk factor of CKD, and whereas HDL-C was the protective factor? that sUA?Morning-evening Difference were the risk factors of CAS and whereasHDL-C?eGFR were the protective factor?that age?morningSBP?CAS?sUA werethe risk factors of LVMI, and whereas eGFR was the protective factor. And the organdamage of anti-hypertension patients is more serious.2.The age?morning DBP?Morning-evening Difference were the independent riskfactors of anti-dipper hypertension, and whereas Pre-awakening Surge was theprotective factor.3.The anti-hypertension was the independent risk factor of CAS?CKD and LVMI4.The diabetes mellitus aggravate organs damage of the anti-dipper hypertension5.In anti-hypertension patients, the morningSBP?TC?LDL-C were the independent riskfactors of patients with anti-dipper hyperten and diabetes;while Rising Surge was theprotective factor.6.Morning-evening Difference was the independent risk factor of CAS?and morningSBP was the independent risk factor of LVMI. Project3: The correlation between F ? gene polymorphism and prematuremyocardial infarctionObjective: To investigate the correlation between F? gene polymorphism andpremature myocardial infarctionMethods: A case control study was adopted, one-hundred premature myocardialinfarction patients in Chinese General PLA Hospital and cooperative hospital and fortycheck-ups patients in Chinese PLA General Hospital were selected. Patients extractedDNA, using direct sequencing assay measured F? gene locus CM084809, rs138388118, rs140370179, CM970462, CM970460, CM050050, rsl801020polymorphism. Clinical data have been collected.
