The Treatment Of The Femoral Neck Fracture In Children

Objective To discuss the therapeutic method of the femoral neckfracture in children.Method Retrospesctive analysis of72cases with femoral neckfracture who were hospitalized in Children,s Hospital of Chongqing MedicalUniversity from January2008to January2014. For no obvious displacementof femoral neck fracture were treated by skin traction?abduction brace wasmade3-6weeks after the femoral fracture. And the children were tried to treatby closed reduction and internal fixiation(CRIF) in displaced fracture,and ifthe closed reduction was failed, we treated them by open reduction andinternal fixiation(ORIF). The comminuted fracture,open fracture and thefracture with severe displacement which may damage the blood supply weretreated by ORIF.Result All72cases of patients were long-term follow-up whichranged from four months to six years and three months(An average of3yearsand4months). According to the evaluation criteria of Ratliff, Of the72patients,53cases(73.6?) had a good result,12cases (16.7?) a fair result and7cases(9.7?) a poor result. With one case of type-? who was treated with conservative occurred in severe coax vara?2cases of type-? occurred insevere avascular necrosis of femoral head?AVN?; With one case of type-?who was treated with CRIF occurred in severe AVN,and one case of type-?occurred in severe coax vara; With1case of type-? occurred in delayedunion and dysfunction due to a pastoperative infection?and1case of type-?occurred in a broken hollow screw.Conclusion With no obvious deplacement and stable femoral neckfracture were treated by conservatively. The children were tried to treat byCRIF in displaced fracture. and if the closed reduction was failed, we treatedthem by ORIF. The comminuted fracture,open fracture and the fracture withsevere displacement which may damage the blood supply can be directlytreated by ORIF.
