The design and implementation of an incident-based safety and risk management information system is based on the consulting project of safety and risk management in a large-scale petrochemical company in China.Firstly, the paper introduced the background of study describing the problems with the current practice on safety and risk management in the company such as non-standard process, incident data disorder and delayed report etc. which asks for the direct requirement for the improvement on safety and risk management and a system to support their safety and risk management. The paper also described the characteristics of production in the petrochemical industry and its risk. The risk involves production material, polytechnic process, equipment and operation which reveal the importance of how to handle these risks in daily production and operation from history data of accidents in petrochemical industry.The paper described the development of theory on safety and risk management and the practice in domestic and oversea petrochemical companies which educe the incident, the incident management and show its importance to safety and risk management in petrochemical industry.Secondly, a study was done on safety and risk management in the large domestic petrochemical company including its organization, production workflow, major accidents and incidents, incident reporting, incident processing procedure and related roles and responsibilities. The system function requirements and non-functional requirements were collected, analyzed and specified into user cases via UML tool using object-oriented analyzing methodology. The function requirements include the incident reporting, incident analyzing, incident handling and incident approving etc… The non-function requirements include such as simple user interface, incident reporting in an unstable network environment etc.According to requirement specifications, the 3 layers architecture was designed for the system including display layer, Business logic layer and data access layer. The system functions have been designed including incident management and system management. Incident management includes 4 modules incident reporting, incident analyzing, incident handling and incident approving. System management includes 2 modules, user management and initial data management. The C# was used to develop the system on.NET platform and the background database is MS SQL 2008 server. All the main functions have been developed such as incident reporting, incident analyzing, incident processing and incident approving etc... The non-function requirements were realized to support system operation as well and all the developed functions were tested and proved. The workflow and component reusability technology were referred and used in system design and development.Lastly, after system was developed, the functions and non-functional features were tested in both development environment and user sites’ environment. Test tool JMeter was used to test the system response performance. The testing result data proved that the system can meet the company’s business requirement of safety and risk management and reach the initial expectation. Now the system has implemented in business unit, divisions, rigs and gone live. The end user gave the good feedback and recognized the system.In the future, the interface of the system is planned to be strengthened to enhance the capability. The system will extend its scope to the preceding business such as audit and inspection which were called as the source activities of incidents. The mobile solution will be considered and developed for the incident management which prepare for a realtime management and meanwhile make it ready for productization.
