With the rapid development of healthcare informatization construction, large amount of clinical data can be more fully utilized by means of clinical data interchange, thereby enabling information interworking between hospitals, optimizing the usage of medical resources, and improving the quality of medical services as a whole. Clinical images contain lots of patient information; if images were intercepted, modified or made public when interchanging, patients’ privacy might be violated. However, existing image encryption methods have some limits and cannot assure security and efficiency at the same time. Symmetric encryption has key management security issues and public-key encryption isn’t fitful for medical image encryption because of its low efficiency.Based on the development status of the image security technology, along with the requirements for medical data interchange and medical information systems, this paper designs and implements a secure medical image interoperation system. In the system, based on the relation between the image header and pixel data, we propose an encryption framework in which pixels and the remaining data are encrypted with AES and RSA separately, and MurmurHash is embedded to ensure image integrity; we uses chaotic sequence to generate AES keys and the improved method is more secure, and suitable for processing large amounts of data because of its support for parallel computing. Based the method we proposed, we design a pattern of public key distribution, develop an image access control module for the EMR system, and record the operation logs, to achieve the control over image manipulation of interoperation systems, thus further ensure the safety of the image data.
