Objective To explore the application value of computer aided planning system in liver volume evaluation, blood vessels evaluation and application of computer aided planning system in liver cancer hepatectomy.Methods CT images of51cases of normal liver and50cases of patients with liver cancer were evaluated by IQQA(?)-Liver system, in order to analysis vessels and evaluate volume. Analysis includes normal liver blood vessels classification, hepatic vein and portal vein main branches basin or perfusion volume and hepatic segment division. Analysis the volume of normal liver, liver with cancer and tumor size. Observe the relationship between the tumor and the structure in liver. Liver surgeons and radiologist evaluated liver CT images of50cases of liver cancer patients by IQQA(?)-Liver system together. Virtual liver resection were applied under "precise surgery" and " precise liver resection" conception.Virtual resection operation (QO) comparing with clinical practical resection operation (SO), the way of virtual vessels broken (QA)comparing with the way of practical vessels broken(SA).Results Normal liver hepatic artery variation rate was13.73%, the hepatic vein variation rate is29.41%, the portal vein variation rate was19.61%. The basin volume of LHV, MHV, RHV and IRHV were252.22±75.03cm3、460.06±114.25cm3、492.72±200.03cm3、265.84±84.19cm3. The infusion volume of LPV and RPV of typeⅠ, LPV, RAPV and RPPV of type Ⅱ were:462.49±81.35cm3、803.53±168.22cm3、455.47±105.12cm3、467.98±54.94cm3.364.82±197.91cm3. Normal liver volume was about1282.91±221.83cm3; volume of liver with cancer was about1710.79±598.84cm3, tumor volume was about451.07±540.41cm3. QO and SO significant correlation (m=0.939, P<0.001), there was no statistically significant difference (P=0.357). QA and SA significantly correlated (u=0.910, P<0.001), there was no statistically significant difference (P=0.750).Conclusion IQQA(?)-Liver system can accurately assess vessels classification and variation, and evaluate liver volume, tumor volume and vessels basin or perfusion volume quantitatively. Preoperative evaluation by computer aided planning system has a certain guiding significance to liver resection.
