The Clinical Report On Diagnosis And Treatment Of 25 Cases Iliac Artery Aneurysms

Objective: To summarize our experiences of diagnosis and treatment of iliac artery aneurysms. Methods: 25 patients with iliac artery aneurysms from October 2011 to December 2015 were retrospectively analyzed the general clinical data, treatment, treatment effect and follow-up of complications, etc. Results:Group consists of 23 cases of male, 2 cases of female, aged 43~85, average(70.2±9.6). Of those 25 cases, 16 cases were treated endovascular treatment, 5 cases were treated open surgery and 4 cases gave up surgery. 21 patients with surgical operation were successful, the patients of preoperative abdominal pain, waist pain, abdominal pulsatile mass and lower limb pain gained significant improvement except 1 case. 1 case without surgery died of aneurysm rupture. The complication of cavity treatment:conveyor stent fracture in 1 case, stent thrombosis in 1 case, stent leakage in 4 cases,2 cases of infection and secondary surgery in 3 cases; Incision infection occurred in 2 cases as an open surgery complication. Endovascular treatment were followed up to 14 cases, 3 cases died, 3 cases of abdominal pain, 2 cases of gluteus claudication, 6 cases of lower limb pain and weakness, 1 case of stent holes and 1 case of aneurysm continued to increase during the follow-up period; Open surgery were followed up to 2 cases, all without any discomfort. Untreated patients were followed up to 1 case, who usually felt abdominal distention discomfort and abdominal pulsatile bag piece still exist. Conclusions:Iliac artery aneurysms should choose individualized treatment. Endovascular treatment can be used as primary surgical treatment for patients with poor basic condition and serious complications intolerance to laparotomy; But for nearly no enough at distal nail iliac artery aneurysms area, severe distortion iliac artery aneurysms and huge iliac artery aneurysms have no cavity treatment conditions, such as open surgery effect is better.
