Objective The aim of this study was to analyze and discuss the clinical diagnosis,the choice of treatment,and the safety and efficacy of isolated iliac artery aneurysm(IIAA).Methods This study collected 28 patients diagnosed as isolated iliac aneurysms from July 2008 to July 2018 at the First Affiliated Hospital of Zhengzhou University.The clinical data of these patients were analyzed,summarized,including basic diseases and preoperative examination.And review,surgical methods,postoperative treatment,perioperative complications and long-term follow-up results,.and of which retrospective analysis had been done.Results We gave a detailed preoperative evaluation for every patient.Personalized adaptive treatment plan was made according to the corresponding examination results.Various vascular interventional instruments and stent graft system were applied during the operation.then we did regular follow-up.3 female patients and 25 male patients were included with an average age of 64.96 years old.Among the 28 patients,7 with lower abdominal pain,16 were asymptomatic,2 urinary system symptoms,and 2 with a huge pulsating mass,and the last one was mainly characterized by swelling of the lower extremities.21 patients underwent endovascular repair for 100% success.One patient experimented emergency surgery and 20 patients reserved elective surgery.The average operation time was(54.1±14.7)min,with no perioperative mortality and(13.1±6.0)ml blood loss.The average hospital stay was(5.9±1.7)days after the operation,2 with internal leakage,and 2 with internal iliac artery ischemia complications.7 patients underwent open surgery for 100% success.One patient experimented emergency surgery and 6 patients reserved elective surgery.The average operation time was(90.7±12.1)min,with no perioperative mortality and(94.3±24.4)ml blood loss.The average hospital stay was(11.3±2.5)days after the operation,1 with poor healing,and 3 with internal iliac artery ischemia complications.We followed all the patients up for 12 to 48 months,with no stent-related complications occurred,while 2 patients with open surgery relapsed.Conclusion The clinical symptom of isolated iliac aneurysms is not obvious.Early effective treatment is needed to prevent from disease expanding and rupture,.Individualized treatment plan is needed due to the location of the aneurysm according to the results of the corresponding imaging examinations.Specific feasible method should be selected according to the diameter and shape.After comparison,it is found that the intravascular endovascular treatment is more feasible and safe,with little trauma and risk,as well as faster recovery and higher feasibility,which can replace the traditional development surgery.
