Relationship And Its Possible Mechanism Between Chronic Left Heart Failure And The Level Of Serum-CA125 In Elderly Patients

Objective: To explore the correlation of the serum Carbohydrate antigen 125(Carbohydrate antigen 125, CA125) level and the elderly with chronic left heart failure, analysis the possible mechanism of serum CA125 level change in elderly patients with chronic left heart failure.Methods: 139 inpatients with chronic left heart failure(Left heart failure, LHF) in the First Affiliated Hospital of School of medicine, Shihezi University were included as case group, and 43 inpatients without chronic heart failure, aged between 60-95 years old. Eligible patients(left ventricular ejection fraction < 50%) in our department were clear diagnosis of chronic left heart failure by more than two or more attending physicians based on diagnostic reference standard Framingham heart failure. All left heart failure patients respectively were grouped according to the New York Heart Association(NYHA) heart function classification method and left ventricular ejection fraction(LVEF). Each basic clinical data, biochemical indexes, echocardiographic data, CA125, precursor protein of B-type natriuretic peptide(NT-Pro BNP) and High sensitivity C reactive protein(hs-CRP) were analyzed by SPSS 17.0.Results:1.Total cholesterol(TC), High density lipoprotein cholesterol(HDL-C) and Uric acid(UA)level in elderly chronic LHF compared with control group, the difference was statistically significant(P<0.05). And TC, HDL-C and UA level were compared among different heart function classification, the difference was statistically significant(P<0.05). LVEF levels decresed and Left atrium diameter(LAD), Left ventricle end-diastolic diameter(LVEDD), End-diastolic volume(EDV), End-systolic volume(ESV) level increased gradually with the increase of cardiac function in elderly chronic LHF, the difference was statistically significant(P<0.05).2.CA125, NT-ProBNP, hsCRP in elderly patients with chronic LHF and the control group was statistically difference(P<0.01), and increased gradually with the increase of cardiac function, the difference was statistically significant(P<0.05). CA125, NT-Pro BNP, hs CRP in the chronic LHF patients with LVEF≤40% were higher than the patients with LVEF>40%,the difference was statistically significant(P<0.01).3.In elderly patients with chronic LHF, CA125 was positively correlated to NT-Pro BNP, hs CRP, uric acid(coefficients were 0.443、0.257、0.295 respectively, P<0.05); The LVEF and CA125 were negatively correlated(coefficients were-0.512, P<0.05); CA125 was positively correlated to LAD, LVEDD, EDV, ESV(coefficients were 0.257、0.352、0.253、0.302 respectively, P<0.05).Conclusions:1.The elderly chronic left heart failure was positively correlated to serum CA125 levels, the serum CA125 level increased with the severity of heart failure.2.The reason of elevated serum CA125 levels in patients with chronic left heart failure may be related to inflammation factors and serous stimulus.
