Objective:To investigate the value of carbohydrate antigen 125(CA125)in the diagnosis and evaluation of heart failure,especially in patients accompanied with renal insufficiency.Methods:A total of 270 patients with heart failure were grouped according to the New York Heart Association(NYHA)heart function classification,and the levels of serum CA125,N-terminal pro-brain natriuretic peptide(NT-proBNP),creatinine and two-dimensional echocardiographic parameters were analyzed to explore the differences of CA125 and NT-proBNP levels in the four groups.The correlation between CA125 and NT-proBNP was analyzed,and the diagnostic value of CA125was evaluated by the receiver operating characteristic(ROC)curve.In the subgroup with severe renal insufficiency[eGFR<30mL/(min·1.73m~2)],CA125 and NT-proBNP were compared in different NYHA classes.ROC curve was also applied for the evaluation of CA125 in diagnosis of heart failure accompanied with severe renal insufficiency.Results:The level of CA125 increased with the elevation of NYHA class(P<0.05),and CA125 was positively correlated with NT-proBNP(r=0.66,P<0.01),negatively correlated with left ventricular ejection fractions(LVEF)level(r=-0.51,P<0.01),positively correlated with left ventricular end diastolic dimension(LVDd)level(r=0.36,P<0.01)in 270 patients with heart failure.The area under the curve(AUC)of CA125 in the diagnosis of heart failure was 0.89(P<0.01).At a cut-off point of 31.66KU/L,the specificity was 92.81%.For the 50 patients with severe renal insufficiency,there was no significant difference in NT-proBNP among the NYHA classes,but the level of CA125 was still increased with the elevation of NYHA class(P<0.05).The AUC of CA125 in the diagnosis of heart failure accompanied with severe renal insufficiency was 0.85(P<0.01).At a cut-off point of45.20KU/L,the specificity was 95.83%.Conclusions:CA125 is significantly associated with NT-proBNP in patients with heart failure,and the combination of NT-proBNP and CA125 can improve the diagnostic specificity of heart failure in patients with severe renal insufficiency.
