| Objective1) To examine the level of hospital stress, Herth Hope and Quality of Life in breast cancer patients during chemotherapy period after operation. 2) To analyze the influence factors of hospital stress in patients. 3) To explore the relationship between hope, stress and quality of life in interviewees.Methods ParticipantsBreast cancer patients during chemotherapy period after operation from three different hospitals were answered by questionnaire by means of random sampling method. At last 208 samples were collected. The participants’ age range from 34~69 years old(50.72±7.96).MeasuresThe questionnaires were asked included demographic information, the Hospital Stress Rating Scale, Herth Hope Index(HHI), Functional Assessment of Cancer Therapy-Breast(FACT-B).Statistical MethodsAll the data were analyzed by SPSS17.0 statistics software. The main methods are: t-test, correlation analysis, and variance analysis.Results1. The scores of the hospital stress, hope and quality of lifeA total of patients’ hospital stress was(95.69±11.05). 99% of them experienced moderate and severe stress totally, 30 cases and 176 cases individually. Patient suffered little stress took up ten percent. A list of stress adversely incidents experienced by hospital patients were: unfamiliarity of surroundings and unadaptable surroundings, loss of little independence, the attitude of staffs and the medical skill, problem with diagnoses and treatments.All of the patients got point range from 30 to 49(37.50±3.67) which standed a relatively high place.And patients who came the high level of hope accounted for 70.19%, while 29.81 percent of patient was intermediate level. 208 patients’ score in quality of life was 89.71±15.73, which also got a high level.2. The influence factors of hospital stress in patientsThe patient who was in a recurrence of illness or metastatic tumor got more severe than their counterpart(P<0.05). The degree of stress that patients underwent was differed in frequency of chemotherapy(P<0.05). The surgical patients after first chemotherapy period perienced the stress compared with patients scoring low(P<0.05). The patients who were in the II stage of tumor, metastasis and node experienced the highest level stress than that of the I,II stage(P<0.05). There was a was positively correlation between the medical insurance and the stress incidents: loss of little independence, financial problems, threat of severe illness and its effects on person, problem with diagnoses and treatments respectively( r=0.107**,P<0.01;r=0.163*,P<0.05;r=0.167*,P<0.05;r=0.240,P<0.05;r=0.210,P<0.05). Education were negatively correlated with threat of severe illness and its effects on person(r=-0.181,P<0.01). Her-2 also were negatively correlated with the stress incidents of Effect individuals and family(r=-0.181,P<0.01).And the income per head in family egatively correlated with the stress( r=-0.181,P<0.01).3. There was a negatively correlation between the stress and quality of life. Therefore, the hope was positively correlated with quality of life.Conclusion1. The severely stress events were experienced by surgical breast cancer patients during chemotherapy period in this study including: no privacy, attitude of staffs, ward with hotter or colder temperature,separation from company, Unfamiliarity of surrounding.2. The factors are recurrence of illness or metastatic tumor, TMN stage, frequency in chemotherapy affected on the hospital patients’ stress. The method payment about medical insurance changed from self-paid to free,simultaneously,the stress went lower. The education background is one of the factors.The patient with higher education background who got lower stress. Her-2 changed from negative to positive and the stress fell down. The income per head reached higher and higher, the stress cut down lower and lower.3. The hospitalization of breast cancer patients who suffered more severely stress, while their quality of life got lower and lower. However, the higher hope in patients, the better quality of life they reached. Most part of the patients believed in beating against the illness, they looked forward to future life. |
