Objectives: The study aims to investigate the infection status of HIV and syphilis among the older female sex workers and to carry out targeted behavioral interventions as well as to provide a basis for preventive measures in the older female sex workers population in Qingdao city.Method: According to the inclusion criteria, subjects were recruited from qualitative research and community-based respondent driven survey in 2013. Unified training investigators with interview experience, including subjects in strict accordance with inclusion criteria, while avoiding repeating subjects in the study, collection and detection of biological samples in strict accordance with the appropriate standards. Both qualitative and quantitative research methods were used to achieve objectives in this study. Individual interviews and group interviews were used in the progress of qualitative research. Meanwhile, community-based respondent driven sampling(RDS) was used, recruiting subjects were completed in strict accordance with the requirements of RDS. Finally, 406 subjects met the inclusion criteria. Included subjects completed the questionnaire, blood samples testing of HIV and syphilis. The self-designed questionnaire also obtains demographic data and various risk factors, risk behaviors associated with HIV and sexually transmitted diseases. After completing the design of questionnaire, researchers assessed the apparent validity and content validity of questionnaire to modulate the entries. The application of qualitative(cognitive interviews) and quantitative(psychological evaluation) technology were used to modify and improve the questionnaire through a pilot study. Epidata3.1 was used to establish a database and complete data entry and double proofreading. All statistical analyses were performed with SPSS19.0. Mean and standard deviation(X ± SD) was used to describe quantitative variable, normality tests using single sample KS goodness of fit, t test was used to compare the difference of means, percentage was used to describe qualitative variable, X2 test was used to compare the difference of P. Univariate and multivariate logistic regression were used. All statistical tests were two-sided, performed at alpha=0.05, the difference was statistically significant when P<0.05.Results: A total of 406 participants are included in this study. The mean age of all the participants is 48.4 years old. The ratio of the Han nationality is 98.3% and most of the participants(73.4%) are from other provinces. According to the marriage state, most of the participants are divorced / widowed population, and the ratio is 62.6%. The second most participants is the married women with a ratio of 34%. The majority of the women have educational level of primary and secondary schools. The junior high school and primary school are following as the second most part, and the ratio are 45.1% and 41.9%, respectively. 17.7% of the participants take drugs more or less. In this study, the rate of HIV infection is 0. However, there are 120 people are detected to be positive syphilis infection with a ratio of 29.6%. Chi-square test of demographic characteristics and syphilis infection behavioral factors and positive syphilis infection are conducted. The results show that significant differences(P <0.05) between age and domicile and whether syphilis infection. Among older female sex workers, whether detected HIV/ syphilis virus is statistically significant(P <0.05) with whether syphilis infection. Univariate logistic regression analysis showed that age, household, personal monthly income(5000-10000 tuple), whether detected HIV/ syphilis virus are statistically significant(P <0.05) with whether syphilis infection. Multivariate logistic regression analysis show that whether detected HIV/ syphilis virus, domicile and whether use drugs have correlation between whether syphilis infection.Conclusion:Age is associated with syphilis infection. The probability of syphilis infection is increasing with the age increased. Registration has an association with syphilis infection. The infection rate of the local people is higher than ecdemic population. The women who has detected HIV/ syphilis virus have relative high rate of syphilis infection. The older female sex workers have high syphilis infection rate in Qingdao city. The risk of infection and the spread of AIDS is also increasing daily.
