Clinical Observation Of Pingwei Capsule Treatment Functional Dyspepsia And An Experimental Study Of Its Effect On CCK And Ghrelin

1 Objective:To observe the clinical effect of Pingwei capsule on patients with liver depression and spleen deficiency of functional dyspepsia, and the expression Pingwei capsule on Cholecystokinin and Ghrelin in plasma and gastric mucosa tissue,and discussion the mechanism of Pingwei capsule treatment functional dyspepsia with Liver Depression and Spleen Deficiency.2 Methods:There were 90 patients who met the standard of diagnosis and the conditions to enter the experiment.They were divided randomly into two groups, 45 patients as the treatment group and 45 as the control group. Treatment group was given Pingwei Capsule(2g at a time, 3times a day), the control group used Mosapride Citrate Dispersible Tablets(5mg at a time, 3times a day), the course of treatment will last for 4 weeks. Observe the total change in efficiency, TCM symptoms integration and gastrointestinal motility, and adverse reactionbefore and the recurrence rates after the treatment. Objectively evaluate the efficacy,gastrointestinal dynamics, safety and recurrence rate of Pingwei Capsule after treatment.Using ELISA immunohistochemical SP method and RT-PCR detection the content and expression of CCK and Ghrelin in plasma and gastric mucosa tissue.3 Results:3.1 Comparison of baseline information There were no statistically significant difference between the treatment group and the control group in terms of gender, age and course of the disease(P>0.05), Two groups are comparable.3.2 Comparison of the efficacy of TCM symptoms Two groups significantly after the treatment and the treatment group is better than the control group(P<0.05), there was a significant reduction in the TCM symptom scores of patients from both the treatment group and the control group after the treatment compared with the scores before the treatment(P<0.05 or P<0.01), and the treatment group is better than the control group(P<0.05).3.3 Electrogastrogram comparison Several parameters of EGG(amplitudes, frequency, fasting power ratio and waveform reaction area)of the first lead(corpus of stomach) and the second lead( antrum of stomach) are significantly improved after the treatment than before the treatment for the treatment group and the control group.On indicators of EGG the treatment group is better than the control group(P<0.05).3.4 Endoscopy and pathological of changed There aren’t any apparent changes which are hyperplasia, intestinal metaplasia, erosion and ulcer for the treatment group and the control after the treatment.Nothing apparent changes of hyperaemia, edema, hemorrhage and inflammation through HE staining of gastric mucosa tissue under microscope.3.5 Content and expression of CCK and Ghrelin Content and expression of CCK decreased significantly, content and expression of Ghrelin increased significantly campared with before treatmentin plasma and gastric mucosa tissue(P<0.05 or P<0.01). Content and expression of CCK between the two groups have no statistically significant(P>0.05), content and expression of Ghrelin between the two groups has a statistically significant(P<0.05).3.6 Safety evaluation Treatment of adverse reaction rates are significantly lower than control group,with a statistically significant(P<0.05);compound was significantly lower in the treatment group,there was significant difference(P<0.05).Conclusion:1 Pingwei capsules has an obvious effect on Functional Dyspepsia caused by liver depression and spleen deficiency. There are noadverse reaction and decrease the recurrence rate, And it can effectively relieve symptoms.2 The mechanism which Pingwei capsule can regulates the gastric motility and mood disorders may be related to change the levels of CCK and Ghrelin plasma and gastric mucosa tissue.
