Analysis On Mental Status In Patients With Chronic Liver Disease

Objective:To investigate whether there is mental health and personality traits difference between patients with chronic liver disease and healthy controls and examine what are the relationships between the severity of chronic liver disease and psychological problems,so as to provide better supports in clinical therapeutics.Eysenck Personal Questionnaire(EPQ)and Symptom Checklist 90(SCL-90)were used in studying these differences between patients with chronic liver disease and healthy controls.Method: Patients with chronic liver disease(Viral hepatitis B/C,Viral hepatitis cirrhosis,primary hepatocellular carcinoma)were selected who receive treatments in digestive department or infection department or department of hepatobiliary surgery in The Second Hospital of Dalian Medical University and digestive department or department of hepatobiliary surgery in Dalian Central Hospital during August 2016 to December 2012.The healthy controls are selected randomly in the Physical Examination Center during the same time.Eysenck Personal Questionnaire(EPQ)and Symptom Checklist 90(SCL-90)are used as assessment scale.Finally,the survey results are analyzed statistically using SPSS22.Results:(1)Basic data analysis: There is no statistic difference in age,gender etc(p>0.05)between patients with chronic liver disease and healthy controls.(2)Analysis of the psychological health(Symptom Checklist 90,SCL-90):(1)There is significant Statistic difference in SCL-90 overall score between patients with chronic liver disease and healthy controls(P<0.05),patients with chronic liver disease have a higher score in somatic symptoms(P<0.05),interpersonal sensitivity(P<0.05),depression(P<0.05),anxiety(P<0.05),others(P<0.05),while healthy controls have a higher score of hostility(P<0.05),phobia(P<0.05)and psychological paranoia(P<0.05),there is no statistic difference in obsessive-compulsive and psychotic factor(p>0.05).(2)There is no statistic difference in SCL-90 overall score between patients with hepatitis and patients with cirrhosis(p > 0.05),but patients with cirrhosis have a higher score in depression(P<0.05)and others(P<0.05),the difference is significant,while patients with hepatitis have a higher score in interpersonal sensitivity(P<0.05),the difference is also significant.There is no statistic difference in somatic symptoms,anxiety,hostility,phobia,psychological paranoia,obsessive-compulsive and psychotic factor(p>0.05).(3)There is significant statistic difference in SCL-90 overall score between patients with cirrhosis and patients with hepatocellular carcinoma(P<0.05).Patients have a higher score in somatic symptoms(P<0.05),obsessive-compulsive(P<0.05),depression(P<0.05),anxiety(P<0.05),phobia(P<0.05),psychological paranoia(P<0.05),psychotic factor(P<0.05)and others(P<0.05)than patients with hepatocellular carcinom.The difference is significant.While there is no statistic difference in interpersonal sensitivity and hostility.(4)There is significant statistic difference in SCL-90 overall score between patients with hepatitis and patients with hepatocellular carcinoma(P<0.05),patients with hepatocellular carcinoma have a higher score in somatic symptoms(P<0.05),obsessive-compulsive(P<0.05),depression(P<0.05),anxiety(P<0.05),phobia(P<0.05),psychological paranoia(P<0.05),psychotic factor(P<0.05)and others(P<0.05)than patients with hepatitis,the difference is significant.Patients with hepatitis have a higher score in interpersonal sensitivity(P<0.05),the difference is significant,however there is no statistic difference in hostility(p > 0.05).(3)Eysenck Personal Questionnaire(EPQ) questionnaire:(1)There is no statistic difference in neuroticism,psychoticism and extraversion between patients with chronic liver disease and healthy controls(p>0.05).(2)There is also no difference in neuroticism,psychoticism and extraversion between patients with hepatitis and patients with cirrhosis(p>0.05).(3)There is statistic difference in extraversion(P<0.05)and neuroticism(P<0.05)between patients with cirrhosis and patients with hepatocellular carcinoma,while there is no statistic difference in psychoticism(p>0.05).(4)As for patients with hepatocellular carcinoma and patients with hepatitis,there is statistic difference in neuroticism(P<0.05)and extraversion(P<0.05),the difference is significant,but no difference is found in psychoticism between the two groups(p>0.05).Conclusion: 1.Compared to healthy controls,patients with chronic liver disease get the main mental health problems in somatic symptoms,interpersonal sensitivity,depression,anxiety,sleep and diet.2.Among the patients with chronic liver disease,patients with hepatocellular carcinoma have more seriously mental health problems than patients with hepatitis and patients with cirrhosis.Specific performance in depression,obsessive-compulsive,anxiety,somatic symptoms,phobia,and psychological paranoia,psychoticism,bad in sleep and diet.Patients with hepatitis have a more sensitive interpersonal sensitivity than patients with hepatocellular carcinoma and patients with cirrhosis.3.There is no significant difference in personality traits between patients with chronic liver disease and healthy people.4.Patients with hepatocellular carcinoma are more likely to be introverted and neuroticism than patients with cirrhosis and patients with hepatitis.
