Hepatitis cirrhosis is a chronic,progressive,diffiuse liver disease caused by the long-term use of hepatitis virus in the liver,with the disease progression,varying degrees of normal liver function.Liver as the center of human lipid metabolism plays an important role in endogenous and exogenous lipid metabolism.The development of hepatitis cirrhosis causes damages to liver physiological function,and it may affect the normal metabolism of human lipids.From the perspective of Chinese medicine theory,the disease is generally attributed to jaundice,hypochondriac pain and other areas.lt is closely related to the occurrence of liver catharsis dysfunction.This study is based on an epidemiological cross-sectional survey.The study analyzes changes of blood lipid in patients with posthepatitic cirrhosis in different degrees of this disease from the perspective of modern medicine.The study analyzes characteristics of blood lipid levels in patients with different disease type syndrome elements from the perspective of traditional Chinese medicine.The purpose of the study is to explore the status and function of liver in lipid metabolism.Part 1 Changes of Blood Lipid Levels in Patients with Different Degrees of Liver CirrhosisObjectiveThe aim of the study is to compare the changes in blood lipid levels in patients with different degrees of liver cirrhosis.MethodsEpidemiological cross-sectional survey method was used to collect patients' clinical information from 9 partner hospital or outpatient.The agencies were responsible for the detection of blood lipid indicators,liver function indicators.SPSS was used to analyze the difference of blood lipid level in decompensated and decompensated patients.Differences in blood lipid levels in patients with different Child-Pugh grades,and correlation between liver function and blood lipid levels were also analyzed by SPSS.Results1.Compensation period in patients with TC,TG,HDL,LDL index values are higher than the compensation,of which two patients' HDL levels are lower than normal.Compensated four blood-lipid in patients with decompensated,and the difference was statistically significant(P<0.05).2.With the rise of Child-Pugh grading,hepatitis and cirrhosis in patients with TC,TG,HDL,LDL targets mean lower shows a,b,c have significant differences of blood lipid index(P<0.05).3.The results of correlation analysis showed that TP,ALB,PA,TBIL,GGT and CHE were correlated with blood lipid index in 14 liver function indexes.Comparison of changes in liver function abnormalities of blood lipid characteristics,the following results were found:When the TP was decreased,the mean values of TC,TG,HDL and LDL were lower than those of TP not elevated group(P<0.05);When ALB,PA and CHE were decreased,the average blood lipids were also lower than those of ALB not elevated group(P<0.05);When TCIL increased,the mean values of TC,TG,HDL and LDL were lower than those in TBIL not elevated group(P<0.05).ConclusionHepatitis cirrhosis patients with the illness increased,impaired liver function also increases,which can lead to lipid lowering.More severe liver damage,lipid lowering more obvious.Part 2 The Relationship between Syndrome Factors and Blood Lipid Index in Patients with Hepatitis CirrhosisObjectiveTo analyze the characteristics of blood lipid levels of different syndromes in patients with liver cirrhosis and liver dysfunction.Methods Epidemiological cross-sectional survey method was used to collect patients' clinicalinformation from 9 partner hospital or outpatient.The agencies were responsible for the detection of blood lipid indicators.Refer to the standard of identification of common TCM syndromes in patients with hepatitis cirrhosis to determine the patient's disease syndrome elements.SPSS was used to analyze different types of syndrome factors of blood lipid levels.Results1.Comparing the relationship between different disease syndrome elements and lipid levels of liver cirrhosis patients,the following results were found:The mean values of TC,HDL and LDL were lower in groups of blood stasis,damp and hot,Yang deficiency,Qi deficiency,and there were significant differences(P<0.05);The mean value of TG was lower in group of Yin deficiency,and the difference was statistically significant(P<0.05);The mean values of TC and LDL were higher in group of Qi stagnation,and the differences were statistically significant(P<0.05);The mean values of TC,TG,HDL and LDL were lower in group of water retention,and the differences were statistically significant(P<0.05).2.Comparing the composition ratio of TCM syndromes in different levels of blood lipid in patients with liver cirrhosis,the following results were found:The proportion of TC-reduced patients in the qi deficiency group(43.2%)was higher than that in the non-qi deficiency group(25.0%).The proportion of TC-reduced patients in water retention group(50.4%)was also higher than non-water retention group(17.6%).The proportion of TG-reduced patients in the blood stasis group(15.6%)was higher than that in the non-blood stasis group(8.5%).The proportion of TG-reduced patients in the yin deficiency group(15.2%)was higher than that in the non-yin deficiency group(10.2%).The proportion of TG-reduced patients in the water retention group(18.5%)was higher than that in the non-water retention group(6.0%).The proportion of HDL-reduced patients in the blood stasis group(66.0%)was higher than that in the non-blood stasis group(56.4%).The proportion of HDL-reduced patients in the yang deficiency group(68.0%)was higher than that in the non-yang deficiency group(56.1%).The proportion of HDL-reduced patients in the water retention group(69.4%)was higher than that in the non-water retention group(53.5%).The proportion of LDL-reduced patients in the water retention group(45.6%)was higher than that in the non-water retention group(32.0%).The difference was statistically significant(P<0.05)ConclusionThe type of syndromes of patients with hepatitis cirrhosis has an effect on blood lipid levels.The results showed that patients in qi deficiency or water retention were more likely to reduce the level of TC.Patients in blood stasis,yin deficiency or water retention were more likely to reduce the level of TG.Patients in blood stasis,yang deficiency or water retention were more likely to reduce the level of HDL.Patients in water retention were more likely to reduce the level of LDL.
