Objective:To examine the short-term effects of salpingectomy on ovarian reserve when ovarian preservation is planned in view of determining the feasibility of conducting the study on a larger scale. Methods:Design: self-reflection, By comparing AMH and Modified Kupperman Index value changes among preoperative and postoperative at 2 months,4 months,et. Patient(s): Seventy-three premenopausal women aged 40 to 45 years undergoing hysterectomy with ovarian preservation forbenign indications from June 2014 to December 2015.Main Outcome Measure(s): Antimullerian hormone(AMH) and Modified Kupperman Index measured preoperatively, at 2 months postoperatively, and at 4 months postoperatively, with operative time and estimated blood loss abstracted from the medical records. Results:(1)The mean AMH levels were not statistically significantly different at baseline,2 months postoperatively or 4 months postoperatively,among women with salpingectomy with laparotomy. “Eore-pulling” salpingectomy were not statistically significantly different,but eonventional salpingostomy were statistically significantly different at baseline,2 months postoperatively or 4 months postoperatively.(2)The mean AMH levels were not statistically significantly different,by comparing uterine size and hemorrhagicanemia or not value changes among preoperative and postoperative at 2 months,4 months.(3)During the operation, there was a significant difference in the detection of pelvic adhesion, concurrent pelvic adhesion lysis(including pelvic endometriosis), preoperative and postoperative AMH value. Conclusion:1.By laparotomy “eore-pulling” salpingectomy during hysterectomy on ovarian reserve in the short term has no significant effect, and by laparoscopic enoventional salpingostomy postoperative ovarian function decline, might be related the way of salpingostomy different, damage to tubal mesentery vascular network in the relevant.2.Through the comparison of uterine size, preoperative whether hemorrhagic anemia had no obvious change, but still could not explain the no effect on ovarian function and need large sample study and longer follow-up survey, in order to better applications, and to guide clinical practice.3.Pelvic adhesion, including pelvic endometriosis, the release of adhesion after ovarian function has been restored, may be related to the pelvic environment, but still need to be further confirmed.
