Objective: To investigate the clinical characteristics of our hospital patients with ulcerative colitis in the past five years, compare the difference between groups of clinical symptoms,laboratory parameters, complications, extra-intestinal manifestations, treatment programs and other aspects, to provide further basis for clinical diagnosis and treatment.Methods: Adopting retrospective analysis method analyze 103 cases of hospitalized patients with ulcerative colitis in the First Hospital of Lanzhou University Department of Gastroenterology from January 2011 to November 2015, their overall clinical features described by the descriptive method,and case-control study method analyze the differences between youth group, middle-aged group and elderly group,and the difference between distal colitis and the pancolitis patients in age,severity of the disease, clinical manifestations, treatment programs, using SPSS software to process and analyze data.Results:1.In all patients, women have a higher incidence of UC(57.28%), average age of patients is52.03 ± 17.18 years old, the high incidence ages are between 40-49 years old, accounting for23.30%; Early onset is a major clinical types( 64.08%), mild accounted for the major part of the severity of the disease(44.66%).2. Grouped according to the extent of disease:DUC is major proportion(DUC) dominated(71.84%).The main clinical manifestations in UC patients are abdominal pain, diarrhea, mucus blood and pus.In aspects of clinical manifestations, complications and intestinal performance, only fever is statistically significant(P = 0.002). The main intestinal manifestations are anemia and arthritis, while skin rash, perianal abscess, cholangitis and so on have low incidence, eye disease is not found. PUC group has a higher proportion of dysplasia, no significant difference between DUC and PUC.3. By age groups:middle-aged is the mainly group(42.72%), patients are mainly DUC in different age groups, but no statistical difference by chi-square test to compare the age groups in terms of the extent of disease; youth group and the elderly group has significantly different(P =0.022) in terms of disease severity by chi-square test analysis.4. According to disease severity groups:mild accounted for the major part(44.66%), nodifferences between genders.There is statistically significant(P = 0.024) between different age groups in terms of disease severity by variance analysis. There are differences between DUC and PUC by chi-square test(P = 0.015): The lesion of patients with mild and moderate are mainly distal colon,while patients with severe type dominate the entire colon.5. According to Mayo index groups:moderately active disease(42.72%) accounted for the major part.Comparing various laboratory parameters by variance analysis: ESR(P = 0.000) and HGB(P = 0.050) has statistically significant between different disease activity groups,they can reflect disease activety; and ESR(P = 0.009) has statistically significant between DUC and PUC groups,the much faster ESR,the more wider range of diseases.6. In the treatment, the main medicine UC patients taked are 5-ASA drugs, DUC patients use local treatment are more than PUC, PUC patientsuse corticosteroids are higher than DUC, only one patient use immunosuppressive agents, 7 cases of PUC patient underwent surgery.7. Surgical patients are mainly male, from mild to middle-aged, duration is short, the majority patients are PUC, and part involvingileocecal, combing chronic intestinal perforationsten, bleeding and other complications.Conclusion: In recent five-years UC patients are mostly women, middle-aged group has a higher incidence, distal colon is the mainlesion,young groupsmainly with severe illness,the older group mainly with mild illness; There was no significant difference in clinical performance between DUC and PUC. The severity of DUC group is mainly mild, while PUC group mainly serious.Younger persons has wider range of diseases. ESR and HGB may reflect disease activity. Treatment method ismainly based with 5-Aminosalicylic acid, and serious patients use glucocorticoids,immunosuppressive agents has a low usage.Surgical patients usually combine with serious complications.
