Objective: Evaluating the related factors,clinical features of patients with pulmonary abnormalities.Method: 70 ulcerative colitis patients complicated with lung abnormalities and 90 controls in Department of Gastroenterology,the Second Hospital of Hebei Medical University,from January 2014 to December 2019,were analyzed.There was no significant difference in gender and age composition between two groups(P>0.05).A retrospective case-control study was employed to compare the clinical characteristics with SPSS 22.0 software.Results:A total of 160 UC patients were enrolled in this study,including 70(43.75%)UC patients with pulmonary abnormalities and 90(56.25%)UC patients without pulmonary abnormalities.1.Among the study group,pneumonia,emphysema,bronchitis,bronchiectasia,asthma,interstitial changes of lung and eosinophilic pneumonia accounted for 60.00%(42/70),18.57%(13/70),11.43%(8/70),11.43%(8/70),2.86%(2/70),10.00%(7/70)and 1.43%(1/70)respectively.13 pathogens were isolated from 11 patients,including 7(53.85%)Gram negative bacteria,2(15.38%)Gram positive bacteria and 4(30.77%)fungi.2.Patients in study group could perform fever,cough,sputum.The incidences of all performances were higher than control group and there was significant difference(x2=12.090,P=0.001;x2=21.961,P=0.000;x2=21.180,P=0.000).3.More patients with length of hospital stay greater than 14 days in the study group,severe disease activity were significantly larger than those in the control group(x2=4.138,P=0.042;x2=8.666,P=0.034),regardless of smoking history,BMI,classification of disease,extent of disease,duration of disease,previous application of 5-ASA,glucocorticoids,azathioprine and infliximab.4.The value of white blood cell(8.09 ± 3.40)*109/L,erythrocyte sedimentation rate(33.09±22.81)mm/h,C-reactive protein level(42.85±51.71)mmol/L and procalcitonin(0.25±0.30)ng/ml in the study group were significantly higher than those in the control group(t=2.801,P=0.006;t=2.808,P =0.006;t=2.889,P =0.004;t=2.177,P =0.033).Conclusion:1.The most common type of pulmonary abnormalities is pulmonary infection.Pathogens isolated from patients including Gram negative bacteria,Gram positive bacteria and fungi.Klebsiella pneumoniae was the most common in Gram negative bacteria.Candida albicans was the most common among fungi.However,in consideration of the small number of sputum culture positive samples in UC patients,it is necessary to expand the sample size for further verification.2.UC patients with length of hospital stay more than 14 days and severe disease activity should be paid attention to pulmonary abnormalities;3.UC patients with pulmonary abnormalities are more likely to have fever,cough,sputum symptoms,higher value of white blood cell,C-reactive protein level,erythrocyte sedimentation rate and procalcitonin.
