| Ulcerative colitis(UC)patients are at increased risk of opportunistic infection due to nutrition decline,immune disfunction and treatment with glucocorticoid and immunosuppressive drugs.As an opportunistic pathogen,Epstein-Barr virus(EBV)infection is related with deteriorated condition and bad respond to therapy in UC.Objective:To clarify the risk factors,clinical manifestations,endoscopic characteristics and the effect of antiviral therapy in EBV-positive UC patients.Methods:1 A cohort of 239 UC patients who were admitted to Department of Gastroenterology in Second Hospital of Hebei Medical University from April2014 to January 2018.2 Viral infection was confirmed by quantitative real-time polymerase chain reaction.Results:EBV prevalence was higher in UC patients when compared with non-IBD controls,especially in those with azathioprine therapy.Meanwhile,the frequency of EBV-DNA was higher in severe UC patients,but had no relationship with gender,age,course of disease and treatment with5-aminosalicylates or corticosteroids.Moreover,patients with fever or abdominal pain were more prone to infected with EBV.Deep ulcer was related with higher prevalence of EBV infection,however the lesion site,endoscopic Mayo score,spontaneous bleeding,platy ulcer and longitudinal ulcer were not correlated with EBV infection.UC with EBV infection had worse prognosis,and antiviral therapy nor therapeutic schedule had no effect on the prognosis.Conclusion:UC is a risk factor for the presence of EBV-positive in blood,particularly in treatment with azathioprine.Patients with fever,abdominal pain or deep ulcer observed under endoscopy should be aware of EBV infection. |
