| Objectives The objectives of this study are to investigate the cognition of the World Health Organization tobacco control strategy MPOWER and to analyze the lack of knowledge about this strategy among medical staff, junior middle school teachers, civil servants in Lanzhou, in order to provide instruction for these people played the role in social tobacco control and speed up the tobacco control work.Methods Medical staff, junior middle school teachers, civil servants were selected by two phase stratified random sampling method, with questionnaire about MPOWER in Lanzhou. The questionnaire included the basic information, the cognition on tobacco control strategy MPOWER,the fulfillment in their respective responsibility about tobacco control and so on. Combined with the MPOWER strategy package respectively selecting evaluation index, to analyze the cognition of this strategy quantitatively.Results 1. The awareness rate of MPOWER strategy was 1.4% in medical staff, junior middle school teachers and civil servants. The fully awareness rate in medical staff, junior middle school teachers and civil servants were respectively 1.6%, 0.8% and 2.1%. The awareness rate of MPOWER strategy was low in objects.2. The cognition of medical staff, junior middle school teachers and civil servants on the series of MPOWER strategy were as follows:2.1 Monitor tobacco use and prevention policies(Monitor)The smoking rate was 17.2% in these objects. The smoking rate of civil servants was the highest, so more attention should be paid to civil servants in future tobacco control work.According to the survey, we found that the smoking places of smokers were usually at home, so the interventions of family and community were necessary. On the cognition of the relevant content in tobacco control work, three groups of people reckoned monitoring the number of smokers and the awareness of the dangers of tobacco, but they were lack of deep understanding about the anti-tobacco advertising work.2.2 Protect people from tobacco smoke(Protect)About the danger of smoking and passive smoking was still in general knowledge on respiratory disease, meanwhile lacking of comprehensive system knowledge. For "breathing clean air is a basic human right", "100% smoke-free environments", "the implementation of theregulations on tobacco control in public places in Lanzhou" three measures, the three groups of people were highly supported, and non-smokers were more than smokers.2.3 Offer help to quit tobacco use(Offer)55.2% of the medical staff often take inquire about smoking situation of patients, 78.8% of them would discourage the smoking behavior of patients and their relatives, and the non-smokers were more than the smokers. The proportions of teachers who taught the knowledge about the danger of tobacco and the smoking refusal skills to their students respectively were 69.8% and41.9%. On the action of stopping smoking behavior of students, the teachers who were willingly to stop the smoking behaviors were accounted for 38.8%, the non-smoker teachers had a higher concept of stopping smoking behaviors than the smokers. 44.7% of civil servants would accept cigarettes from other people, but only 11.0% of civil servants would dissuade others’ smoking behaviors. The analysis of the influence factors on the consciousness of offering smoking cessation in three groups of people showed that participating in the training on dangers of tobacco related knowledge was the key factor that affected their consciousness.2.4 Warn about the dangers of tobacco(Warn)Most people thought the current cigarette packets warnings were not effective, they hardly would notice them. We found that more than 50% of people thought the warnings on cigarette packets should be accounted for a third. More than 60% suggested that black lung, rotten teeth and other warning pictures should be used on cigarette packets, which could play more important roles in the warning effect.2.5 Enforce bans on tobacco advertising, promotion and sponsorship(Enforce)5.3% of the medical staff fully aware of the relevant provisions of the tobacco advertising in the advertisement law, junior middle school teachers and civil servants were 1.6% and 5.9%. More than 60% thought the tobacco sales putting on display would enticed teenagers smoking, and on the cognition of tobacco sponsorship, 50% of people would accept the tobacco companies fund supplies.2.6 Raise taxes on tobacco(Raise)52.7% of respondents believed that the number of smokers would reduce after raising price,and most people approved of raising taxes on tobacco control, only a few didn’t agree with this methods.3.According to the cognitive quantitative scores of MPOWER strategy in three groups of people. We found that there were certain biases on the tobacco warnings and tobacco sponsorship in medical staff; junior middle school teachers were lack of awareness of stopping other people from smoking, the awareness was also need to be improved in civil servants. The cognitive scoresin medical staff’s were obviously higher than in the junior middle school teachers or the civil servants(P<0.05).Conclusions The awareness of the tobacco control strategy MPOWER among medical staff,junior middle school teachers, civil servants in Lanzhou was low. According to the MPOWER strategy package analysis, the cognition of protecting people from the danger of smoking and the approval of raising the taxes of tobacco were higher, but lower in offering smoking cessation in three groups of people. For the cognition on tobacco sponsorship, advertising still need to be improved. The results showed that improving the effect of tobacco control should attach importance to their own understanding of tobacco control strategy firstly, and relevant departments should strengthen the targeted health education. secondly, on the tobacco behavior intervention, improving the cognition on the dangers of tobacco and carrying on the skills training of discouraging smoking behavior are important. Thirdly, we should pay attention to the good social atmosphere and environment construction of tobacco control, such as tobacco advertising,promotion and sponsorship, the strict law and guides should be enforced. |
