Objective The effect to different types of intraocular lens’ tilt and decentration after Nd:YAG laser posterior capsulotomy and the visual quality of posterior capsule opacification.Methods 1. A prospective comparative study. 112 eyes of 95 patients with clinically observable posterior capsule opacification,including group A;3-pieces hydrophobic acrylic,C-shaped haptics intraocular lens Tecnis 9003(ZA9003)and Sensar(AR40e) 28 eyes,group B:1-piece hydrophobic acrylic,C-shaped haptics HQ201 hep XO、HQ201hep 30 eyes,group C:1-piece hydrophobic acrylic,3-haptics CT ASPHINA 603P(ZO)22 eyes,group D:1- piece hydrophobic acrylic,4-haptics BauschδLomb ADAPT-AO(BL-AO)32 eyes,who scheduled to have Nd:YAG laser posterior capsulotomy at our clinic,were included in this study。Before and after one month after Nd:YAG laser posterior capsulotomy, fully mydriatic,using slit-lamp digital camera to take picture on the posterior capsule and using Pentacam to get the information of the anterior segment,using the image pro plus 6.0 software to get the pupil diameter, tilt and decentration of IOL,using the image J software to get the P score at 5mm diameter. 2.50 eyes of 44 pseudophakic patients with clinically observable posterior capsule opacification(fibroelastic type, Ⅰ-Ⅱdegree),were included in this study.Using slit-lamp digital camera to take pictures of the posterior capsule,getting the pictures of the anterior segment by Pentacam,using the image J software to get the P scores at the 4mm diameter of the pupil. The OSI, SR, MTF cut off、 OVs were measured at the 4mm diameter of the pupil by OQAS.Results 1.No significant difference of the age(67.32±16.80、64.14±12.21、68.50±11.00、66.65±11.05) between four groups,F=0.371,P=0.774>0.05. No significant difference of the diameter of laser hole(4.355±0.206、4.336±0.226、4.392±0.210、4.413±0.177) between four groups,F=0.648,P=0.586>0.05,There was correlation between the P scores(34.025±14.804) and the tilt(1.863±1.716) of IOL,the P scores and the decentration(0.280±0.157)of IOL before the Nd:YAG laser posterior capsulotomy(r=0.428、0.257, P=0.734、0.303).There was significant difference of the tilt(1.863±1.716、0.986±0.548)and decentration(0.280±0.157、0.165±0.010)before and after the Nd:YAG laser posterior capsulotomy(P<0.05).Pairwise comparisions, there was a significant differences between the group A and group B,group A and group C,group A and group D,group B and group C,group B and group D(P<0.05),no difference between group C and group D(P>0.05). 2.After Nd:YAG laser posterior capsulotomy,the BCVA improved. There was correlation between the P scores and the OSI、MTF cut off、SR(P<0.05).Significant differences of the OSI、MTF cut off、SR、OVs before and after the Nd:YAG laser posterior capsulotomy(P<0.05).Conclusions 1. The tilt and decentration of IOL was reduced after Nd:YAG laser posterior capsulotomy, tilt and decentration of 1-piece IOLs are less than those 3-piece IOLs,tilt and decentration of the 3-haptics and 4-haptics IOLs are less than C-shaped haptics IOLs. 2.The opacifiction of posterior capsule can reduce the visual quality,PCO has great influence on visual quality.The visual quality improved after Nd:YAG laser posterior capsulotomy.
