| Objectives We are aim to investigate the quality of life,the degree of anxiety,the degree of depression,and the level of the self-management-efficacy after the eradicative resection of breast cancer patients during the chemotherapy. In the observation group patients,we used the self-management mode of nursing intervention to explore the selfmanagement mode influence of the quality of life,the degree of anxiety,the degree of depression,and the self-management efficacy in breast cancer radical surgery patients, those who accepted chemotherapy. We are aim to explore a set of effective nursing measures of self-management for postoperative patients with breast cancer,in order to improve the life quality of breast cancer patients and to achieve the self management goal,further more to promote physical and mental rehabilitation of patients.Methods Our intervention study used the random sampling method,the samples were postoperative patients with breast cancer obtained form the Affiliated Hospital of Chengde Medical College during the period of April 2014 to March 2015. These postoperative patients accepted TEC chemotherapy(Taxotere T + Epirubicin E + Cyclophosphamide C).According to the principle of random grouping,the patients were divided into the control group and the intervention group. The control group patients were given regular breast cancer postoperative nursing and health education, the patients in intervention group were given not only the regular nursing and health education,but also given selfmanagement intervention through six cycles of chemotherapy. We collected data by the method of questionnaire survey before and after the intervention,we compared the patient’s life quality,degree of anxiety,degree of depression,and the self-management efficacy between the two groups at the point of before the intervention,the end of one course chemotherapy,the end of the third course chemotherapy and the end of six course chemotherapy(that is,the early,the middle and the late period),then analyzed the effect of the self-management intervention. Survey tools included general situation questionnaire,the Functional Assessment of Cancer Therapy-Breast(FACT-B,version 4.0),the Anxiety Self Rating Scale(SAS),the Depression Self Rating Scale(SDS) and the Cancer patient self-management self-efficacy questionnaire(SUPPH). We used SPSS19.0 statistical software to analyze the data, statistical methods included the statistical description,the t-test,the chi-square test etc. The measurement data( sx ±)were analyzed by the t-test,and the count data of the frequency and rate(%)were analyzed by the chi-square test(P < 0.05)was considered to indicate statistical significance.Results 1 In the two groups,there are no correlation in age,the cultural degree,the marital status,the family relationship,the main caregivers,family income,the type of medical security,the clinical staging and the operation method etc(P > 0.05).2 Before intervention,there are no correlation in the patients’ quality of life, the degree of anxiety, the degree of depression, self-management efficacy between the control group and intervention group(P > 0.05).3 After intervention of one course chemotherapy,three course chemotherapy and six course chemotherapy,the inter-group comparison exhibited a significant correlation with the patients’ quality of life,the degree of anxiety,the degree of depression and self-management efficacy between the control group and the intervention group(P<0.05). 4 The inter-group comparison was made to compare the patients’ life quality, the degree of anxiety,the degree of depression and self-management efficacy between the two groups before and after the intervention.(1) The control group: At the end of the one,three and six course chemotherapy, the baseline of the patients’ life quality,the degree of anxiety and the degree of depression were slightly lower,the scores of self-management efficacy were slightly higher compared with before intervention,but there were no statistical significance(P > 0.05).(2) The intervention group: At the end of the one, three and six course chemotherapy,the baseline of the patients’ life quality,the degree of anxiety,the degree of depression and the scores of self-management efficacy were obviously higher compared with before,the difference has statistical significance(P<0.05).Conclusions Through self-management interventions to improve the life quality of postoperative patients with breast cancer; Reduce the anxiety in patients with depression, and longer duration of intervention in patients with anxiety depression degree gradually reduce; Improve the level of the patient self-management self-efficacy, and longer duration of intervention self-management self-efficacy level gradually improve; Selfmanagement is an effective intervention mode,and is worth to widely apply in the rehabilitation of postoperative patients with breast cancer. |
