| Objective:Exploration of case management model to adapt to the improvement of the ability of breast cancer patients’ quality of life and social psychology, the establishment of the whole case patients with breast cancer management mode, to provide reference for the establishment of a comprehensive mode of case management practice.Methods:From January to 2014 were randomly selected from 2014 September in Hunan Provincial Tumor Hospital breast oncology center hospital met the inclusion criteria 130 patients with breast cancer patients. Were randomly divided into intervention group and control group, each group of 65 cases.The control group implemented routine management of breast cancer, the intervention group received case management model,according to the characteristics of breast cancer patients and the postoperative quality of life changes, formulate individualized case management scheme, in the implementation process, constantly revised case management program, to complete the assessment, project management plan,implementation, evaluation, feedback. Before and after the intervention were used for breast cancer patients quality of life scale and psychosocial adaptation questionnaire of quality of life, psychological and social adaptation ability of disease intervention group and control group, the establishment and improvement of breast cancer case management program, all data are used SPSS18.0 statistical analysis software, the main statistical methods including descriptive analysis, chi square test, non parametric test, two independent samples t test.Results:1. Before the intervention, the intervention group and control group, quality of life and psychological disease score comparison of social adaptation. The intervention group quality of life and disease psychosocial adaptation total score was 44.90 + 8.821, 58.47 + 6.185, the control group quality of life and disease psychosocial adaptation total score was 46.19+ 10.209, 57.31 + 9.303, the control group, the intervention group before intervention, quality of life and social adaptation disease score comparison, the difference was not statistically significant, P > 0.05, respectively F =0.007,P=0.931 and F=1.806, P=0.184.2. The intervention group before and after intervention compared to before intervention the score of quality of life QOL total score 44.90 + 8.821, the dry mass of survival in a total score of 49.63 + 6.724, the score of quality of life in the intervention group compared before and after the intervention, t =-2.279, P, =0.030, the difference was statistically significant.Before and after the 3 intervention group psychosocial adaptation score compared with before intervention disease psychosocial adjustment a total score of58.47 + 6.185, 70.33 + /- 4.859 total score of stem to prognosis of psychosocial intervention group before and after the intervention, the disease of psychosocial adaptation scores were t=-8.139, P= 0, the difference was statistically significant.4.In the control group before the intervention and 6 months after intervention quality of life scores of quality of life before the total score 46.19 + 10.209, 6 months after the total score of quality of life of 43.25 + 13.800, the score of quality of life compared to control group before intervention and after6 months, t = 1.110, P =0.276, no significant difference.5. In the control group before intervention and 6 months after the disease psychosocial adaptation scores before interventiondisease psychosocial adaptation total score 57.31 + 9.303, the control group after 6 months of psychosocial adaptation of total score of 53.19 + 12.985, control disease psychosocial adaptation scores between the groups before and after 6 months,t = 1.629, P = 0.113, the difference was not statistically significant, P > 0.05.6. The intervention group patients quality of life and disease psychosocial adaptation total score was 49.63 + 6.724, 70.33+ 4.859, group intervention on quality of life and disease psychosocial adaptation total score was 43.25 + 13.800 and53.19 + 12.985, the control, the control group, the intervention group after the intervention, two groups of quality of life, psychological and social adaptation disease scores were compared with F=9.490, P=0.003 and F=27.322 P=, 0,the difference was statistically significant.Conclusion:1. the entire case management service mode can improve breast cancer patients postoperative quality of life;2. the entire case management service mode can improve disease psychological social adaptation ability;3. case management service mode in the whole process for implementation in the patients with breast cancer. |
