Objective:To explore the relationship between severity and local complications of acute pancreatitis after conservative treatment.Methods:Medical records were reviewed and analyzed of 2227 AP patients who were admitted in chongqing from february 2010 to February 2015 in terms of incidence and severity of local complications.Result:Gallstone(n=1137,51.0%),hyperlipidemia(n=741,33.3%), alcohol(n=349,15.7 %).Local complication rate is 39%, biliogenic pancreatitis 43.0%, hyperlipemic pancreatitis 36.3%, while alcoholic 31.6%.The incidence of local complications in Gallstone AP is higher than the other two groups, hyperlipemic pancreatitis and alcoholic pancreatitis have no differences in the incidence of localcomplications.According to the number of positions the pancreatic leakage refers to, the complications can be classified into three stages.Gender,age have no statistical difference(P>0.05) in the severity of local complications.The local complications in Gallstone AP is more serious than metabolic AP(P = 0.000) and alcoholic AP(P = 0.015). Metabolic AP and alcoholic AP has no obvious differences in the severity of local complications(P>0.05).Conclusions : After conservative treatment, the incidence of local complications of biliogenic AP was the highest. In biliogenic AP patients,severe local complication rate was the highest.Biliogenic AP is easier to result in a more serious clinical process. so in the clinical treatment of biliogenic AP should be timely relieve biliary obstruction and not just to intervene in case of inflammatory exudation.
