Acute pulmonary embolism is a common clinical emergency disease which is associated with high morbidity and mortality. Pulmonary embolectomy has been used as an emergency life-saving treatment for acute pulmonary embolism, and it has been always accompanied by controversy. This article was a review of published literatures on pulmonary embolectomy in order to explore the clinical status. The results showed that the pulmonary embolectomy mortality, in recent years, had a downward trend with the cumulative mortality rates were 15.3%(2010 ~ 2011), 12.6%( 2012 ~ 2013), and 6.6%(2014 ~ 2015). It was further confirmed that pulmonary embolectomy had better clinical outcomes. Compared with thrombolytic therapy, pulmonary embolectomy had a downward trend in early mortality, postoperative bleeding and recurrent PE complications. Preoperative hemodynamic status and cardiac arrest could affect the prognosis of pulmonary embolectomy significantly. With further understanding of acute pulmonary embolism and the development of medical technology, more and more pulmonary embolectomies had been performed in many medical centers, and early surgical embolectomy had been emphasized before the hemodynamic and circulatory collapse among the patients with acute pulmonary embolism whose hemodynamic stable but with right ventricular dysfunction. Pulmonary embolectomy has been an effective treatment for severe acute pulmonary embolism, patients with acute pulmonary embolism can get better benefit from the procedure.
