Objective To investigate the factors affecting the efficacy of arthroscopic debridement in the treatment of knee osteoarthritis.Methods Collecting 258 patients with knee osteoarthritis treated with arthroscopic debr idement which according with inclusion and exclusion criteria in our hospital from Jan uary 2009 to December 2013.Record the patient’s details,includeing the patient’s gender, age,genetic factors,race,occupation,body mass index,duration of the lesion(left/right),psyc hological factors,smoking history,drinking history,preoperative X-ray Kellgren-Lawrence former grade,preoperative VAS pain score, preoperative Lysholm score and postoperativ erehabilitation exercise.Patient follow-up after six months and one year, gave with VA Spain score and Lysholm score.Assessing knee function at last follow-up with Lysholm and VAS pain score.Useing SPSS 17.0 statistical software for analysis, count data using (x|-)±s measurement and ranking data using χ~2 test.Every factor first using single facto ranalysis.Univariate analysis was statistically significant factors then Logistic multivariate regression analysis.P<0.05 indicates significant difference.Results A total of 202 cases (224 knees) were followed up for 12-18 months,an avera ge of 14 months.Of which 70 males(34.7%),female 132 cases(65.3%),224 knee(on the right 122 cases,102 cases of the left),follow-up rate of 78.3%.The patients were followed uniform healed without infection,deep vein thrombosis and other complications,according to the Lysholm score,excelent group of 175 cases (56 cases excellent+199 cases good),accounting for 78.1%;difference in group 49 cases(33 cases less than good+16 c ases poor),accounting for 21.9%.Single factor analysis showed that age,course of disease,body mass index,psychological factor,preoperative VAS score,preoperative Kellgren-La wrence grade,preoperative Lysholm score,postoperative rehabili-tation exercise affected the efficacy of knee arthroscopy in the treatment of knee osteoarthritis,the difference be tween the two groups was statistically signifycant.Further multivariate analysis showed that high body mass index,preoperative VAS pain score,Lysholm score was low,and high psychological expectations,preoperative Kellgren-Lawrence grading is expected to be the independent risk factors of knee arthroscopy in the treatment of knee osteoarthritis.Compared with the Lysholm score,single factor analysis according to the VAS score only the course of disease is different and multivariate analysis is the same.Conclusion High body mass index,high preoperative VAS pain score,low preoperative Lysholm score,high preoperative Kellgren-Lawrence grading,high psychological expectati ons is expected to reduce the effect of surgery,postoperative strict compliance with rehabilitation exercise can improve the curative effect.
